FTE: .4, Shift: Day, Work week Monday-Friday Title Position: Public Health Nurse (PHN) ROLE SUMMARY: The Public Health Nurse provides comprehensive nursing services to high-risk, vulnerable families with emphasis on women, infants, and children: and facilities the attainment and/or maintenance of the greatest possible physical, psychological, and social well-being of their clients. ESSENTIAL RESPONSIBILITES: Conducts systematic nursing assessments of client's strengths and needs using a combination of appropriate program assessment tools. Develops an individualized Nursing Care plan with the client based on medical and social factors, health behavior, and need for health/parenting education identified in the nursing assessment. Goals are mutually set with the client. Provides teaching and counseling adapted to the unique strengths and needs of clients and families. Orients new clients to program services, including purpose of home visits. Maintains adequate contact with clients; evaluates effectiveness of interventions; documents outcomes. Demonstrates advocacy for client/family. Accept, reviews, and assigns to appropriate department/agency, if applicable. Collaborate with Mandatory Reporting Law for reporting any suspicion or incident of child abuse or neglect within 48 hours, if applicable to department. Participates in team meeting and case conferences. QUALIFICATIONS AND REQUIREMENTS: Education: BSN Required. Experience: Perinatal/Lactation and/or Pediatric experience. License/Certification/Registration: Washington State (DOH) Registered Nurse Licensure required. Bilingual skills (English/Spanish) preferred Position Summary The RN completes individualized patient assessments; develops a care plan based on assessment/history findings; delivers patient-centered care that is culturally appropriate; evaluates a patient's response to care and revises as needed. The RN is accountable to deliver care as well as to oversee and delegate to other personnel (clinical & non-clinical) in delivering/supporting the delivery of care. ESSENTIAL RESPONSIBILITIES 1. Collects and analyzes pertinent objective and subjective data related to the health status of the patient/client, including biological, psychological, social, emotional, spiritual, and age-specific determinates of health. 2. Plans, implements, and documents plan of care involving family, demonstrating respect for patient rights. Maintains patient privacy and confidentiality; is compliant with HIPAA. 3. Advocates for patient/family. 4. Honors, respects patient/family cultural beliefs and incorporates into plan of care. 5. Communicates plan of care to members of the healthcare team. 6. Delegates selected nursing functions to others in accordance with their education, credentials and demonstrated competence. 7. Evaluates and revises plan of care to address physician/clinical issues. 8. Identifies care concerns and communicates with appropriate individuals or departments to ensure resolution. 9. Knows responsibility/role during all Emergency Codes and Red Rules. 10. Maintains current clinical knowledge and volunteers to share/present information to staff. 11.
