Virginia Mason Memorial Registered Nurse (RN) - Outpatient Clinic Triage and Care Coordination in Yakima, Washington
ROLE SUMMARY The RN, Outpatient Clinic Triage and Care Coordination role is multifaceted. As the Clinic Triage RN, this role is responsible for facilitating the flow of patients, ensuring that care is appropriate and well-understood by patient and family alike. The RN Outpatient Care Coordination role is responsible for the development, implementation, monitoring and revision of a comprehensive care plan for Medicare-enrolled patients who are referred for Chronic Care Management by a Provider.
QUALIFICATIONS AND REQUIREMENTS Education: Associate’s degree in nursing required; BSN preferred. Experience: Five years’ acute care nursing experience required. Case Management experience preferred. Licenses/Certificates/Registration: Washington State (DOH) Registered Nurse licensure required. American Heart Association BLS required until certified in ACLS. ACLS required within one year of hire (if applicable).
