Responsibilities: Provide family-centered, home-based services Conduct the enrollment, service planning and implementation of the Parents as Teachers Curriculum focusing on strengths and areas of concern. Conduct home-visits, addressing specific goals in the family service plan; conduct developmental screenings; make referrals and establish connections to other community resources identified in the service plan; facilitate educational groups. Document services provided to the family and submit required paperwork in a timely well-organized fashion. Participate in PAT staff meetings, multi-disciplinary team meetings, and other meetings as required. Follow agency policies regarding confidentiality and other consumer rights Maintain flexible hours and schedule visits according to families needs Represent the agency in a professional manner within the community Work well with other agencies personnel Requires working flexible hours, including some evenings a month Requires extensive travel in the community and working in clients homes Requires ability to tolerate exposure to tobacco smoke, domestic animals, household pests, and other conditions common to a variety of home environments Perform other duties as required Job Requirements: The following requirements are those that are normally required for performance of this position. Non-Physical Requirements: Education Bachelors Degree in Behavioral Sciences, Preferred Associates Degree, Required Experience 2 years experience working with parents and young children 5 years experience working with parents and young children with Associates Degree or equivalent Special Skills Knowledge of child development, a range of child rearing practices and issues Knowledge of community resources Knowledge of appropriate use of screening materials Ability to work independently and as a team member Possess good verbal and written communication skills Possess good data management skills Bilingual (English/Spanish) Required Ability to provide culturally relevant services Licensure, Registration, Certification First Aid/CPR certification requi
