Summary The incumbent serves as a Social Science Program Coordinator - Whole Health Coordinator and is responsible for the delivery of Whole Health coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching in promotion and education regarding Whole Health coaching to Veterans and staff. Responsibilities The position is integral to supporting the Chronic Pain Management Program with Whole Health concepts and coaches identified Veterans in navigating the whole health journey, specifically through the pathway, wellbeing, and Whole Health clinical care elements of the Whole Health system, as well as assisting in the collection of Whole Health qualitative data from Veterans and creation of reports analyzing effectiveness of health coaching. This position works with individuals identified through the Pain Clinic process and utilizes a client centered process to facilitate and empower the client to develop and achieve self determine goals related to health and wellness. Coaches support client in mobilizing internal strengths and external resources and develops self management strategies for making sustainable, healthy lifestyle behavior changes in more effectively managing chronic disease. The following duties are performed: Contacts eligible patients for enrollment and explains the health coaching process. Provides health coaching for Veterans in order to reduce or illuminate high-risk behaviors while increasing healthy ones. Conducts interventions for Veterans using one on one, couple, or group modalities in outpatient settings including live face to face and virtual services. Conducts whole health facilitated groups; participates in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other outpatient groups as identified by the Pain Team. Provides follow up care via telephone clinics and secure messaging between face-to-face appointments. Actively guides Veterans in the exploration of core elements of personal health planning through individual and group based programs. Encourage and assists Veterans with the adoption of habits that are conducive to a higher quality of life and in alignment with the veterans goals. Identify habits and behaviors that are harmful to quality of life and related to chronic illness. Reviews current behaviors and develops customized treatment plans with Veterans. These plans include personalize goals, action steps and interventions. Individualized intervention strategies to help the client utilize existing skills and develop new ones to enhance success. Schedule on-going sessions to assess progress and identifies obstacles that may be challenges to success. Guide Veterans to learn about available support tools in the VA in the community that assist in attainment of goals. Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. BASIC REQUIREMENT: You MUST meet the Basic Requirements and the Minimum Qualifications to qualify for this Social Science Program Coordinator position. Degree: Behavioral or social science; or related disciplines appropriate to the position.. OR Experience: Four years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. OR Combination of education and experience: That provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. Combinations of successfully completed graduate level education and specialized experience may be used to meet total experience requirements. Only graduate level education in excess of the amount required for the next lower grade level may be combined with experience. For example, an applicant with 6 months of appropriate experience equivalent to GS-7 (50 percent of the experience requirement for GS-9) and 27 semester hours of appropriate graduate education (50 percent of the education requirement for GS-9, in excess of that required for GS-7) would be qualified for a GS-9 position. NOTE: If you qualify by a combination, transcripts are required, and your experience must be detailed in your resume. In addition to meeting the above requirements, you must also meet one of the following minimum requirements for the grade level that you are applying. To qualify for this position at the GS-7 level, you must meet one of the following: Specialized Experience: You must have at least one (1) full year of specialized experience equivalent to at least the next lower grade level (GS-5) in the Federal Service, that is directly related to the work of the position and has equipped you with the particular knowledge, skills and abilities to successfully perform the duties as a Social Science Program Coordinator. Qualifying specialized experience includes: promote health and wellness, improve health-related outcomes, reduce likelihood of impatient admissions, and improve quality of life. Conducts interventions for patients, develops treatment plans for veterans focused on behavioral interventions, etc. OR Education: 1 year of graduate-level education or superior academic achievement OR Combination: Completion of graduate level education in the amounts shown in the table, in addition to meeting the basic requirements, is qualifying for positions at grades GS-7 through GS-11, if it provided the knowledge, skills, and abilities necessary to do the work. One year of full-time graduate education is considered to be the number of credit hours that the school attended has determined to represent 1 year of full-time study. If that number cannot be obtained from the school, 18 semester hours should be considered an academic year of graduate study. Part-time graduate education is creditable in accordance with its relationship to a year of full-time study at the school attended. To qualify for this position at the GS-9 level, you must meet one of the following: Specialized Experience: You must have at least one (1) full year of specialized experience equivalent to at least the next lower grade level (GS-7) in the Federal Service, that is directly related to the work of the position and has equipped you with the particular knowledge, skills and abilities to successfully perform the duties as a Social Science Program Coordinator. Qualifying specialized experience includes: reviewing supporting material to determine compliance with regulations and policy requirements; assists in planning and implementing program(s) within a medical center setting; using creativity to engage Veterans and community partners in a newly developed program; developing training materials for staff educational needs; and experience using a wide range of qualitative and quantitative methods for the assessment and improvement of program effectiveness. OR Education: Successfully completed two (2) years of progressively higher-level graduate education leading to a master's degree or master's or equivalent graduate degree in a related field of the position to be filled. Education at the graduate level must have been obtained in an accredited college or university and must demonstrate the knowledge, skills, and abilities necessary to do the work of this position. OR Combination: Combinations of successfully completed graduate level education (beyond the first year) and specialized experience as described above, to meet the total experience requirements. The education portion must include graduate courses that demonstrate the knowledge, skills, and abilities necessary to do to the work of this position. Physical Requirements: While some of the work is sedentary including researching, writing, program planning and data analysis, the position will have periods of extensive walking and movement throughout the healthcare system, prolonged standing, and transporting light to medium weight objects. The Employee Whole Health coordinator will also be required to travel between sites (CBOCs and outlying clinics) encompassed in the healthcare system. The work is conducted in a medical center environment and involves risk of exposure to a variety of contagious diseases. The work area is adequately lighted, heated, air conditioned, and ventilated.
