The System VP Clinically Integrated Network leads the development and implementation of the organization's programs and services designed to improve the health of contracted populations served, creates seamless care transitions, and manages total costs of care. Responsible for evolving clinical care coordination structures to optimize clinical care and patient experience while enhancing value that drives service, experience and maximizes the use of assigned resources. Oversees clinical care coordination across the continuum for identified populations and designs clinical programs that ensures alignment of the PeaceHealth physician and provider community and creates performance structures that drives accountability and achievement of the organization's goals and objectives. Specific focus is on management of total cost of care, enhancing domestic utilization, care transition, and patient satisfaction. Supports, through collaboration, PeaceHealth's integrated network and the development and growth of its value-based contracts ensuring excellence in clinical, quality and utilization performance and population health outcomes. Manages through direct accountability and influential leadership and holds a strong knowledge of population health, healthcare operations (acute, ambulatory, and post-acute), care management structures and the applications that support targeted outcomes, inclusive of quality, safety, experience, and stewardship. Leads efforts to reduce variation and integrate standard work where applicable.

ESSENTIAL FUNCTIONS

  1. Through collaboration with senior leadership, develops and implements the overall strategy for clinical care coordination in alignment with values-based care objectives.

  2. Develops the clinical care coordination roadmap for values-based care to achieve PeaceHealth‘s vision: Every person receives safe, compassionate care; every time, every touch with the development and deployment and population health strategies and interventions.

  3. Establishes clear goals and operating targets to ensure leadership accountability to metrics and key performance indicators align with system goals including performance in targeted total medical and pharmacy costs.

  4. Ensures collaboration of PeaceHealth leaders across the continuum to effectively and efficiently address care transitions that reduce the total cost of care, improve clinical delivery, and enhance the patient experience.

  5. Designs, implements, and oversees care coordination functions across the acute, ambulatory, and post-acute continuum tailored to value-based care models. Ensures programs are patient centered, efficient, and effective in improving health outcomes and managing costs.

  6. Leverages health information technology to improve care coordination. Stays current on necessary and innovative technology and participates on the Value Based Care Governance Technology committee to ensure resources are properly aligned and support the delivery of efficient and seamless care.

  7. Collaborates and supports Finance leadership to negotiate and manage value-based contracts with Commercial and Government Payors.

  8. Partners with physician leaders to ensure physician partners are integrated and aligned. Creates collaborative structures and ensures strong and effective communication with physician and the clinical care team.

  9. Identifies and mitigates risks associated with care coordination and value-based care.

  10. Performs other duties as assigned.

QUALIFICATIONS

REQUIRED EDUCATION

  • Bachelor’s degree in Nursing, Business, Public Health Administration, or Hospital Administration.

  • Master’s degree in Nursing, Business, Public Health Administration, or Hospital Administration.

REQUIRED EXPERIENCE

  • Minimum of 10 years leadership experience in a complex system that has focused on physician, clinical integration, network operations and population health strategies.

  • Minimum of 5 years experience as a healthcare executive.

  • Prior experience and successes in a system executive leadership role with demonstrated outcomes in an integrated network.

  • Experience leading/partnering in multidisciplinary teams.

  • Ability to adequately express and sell new concepts and ideas through influence and persuasion.

  • Diplomatic as well as organizationally savvy, able to relate to and open to differences; presents views and arguments well; able to drive change management initiatives and build diverse networks that align and support PeaceHealth's mission and vision.

  • Collaborative team player with high integrity and ethical standards.

  • Possesses strong problem-solving skills with the ability to facilitate the resolution of conflicts and grievances, handling highly charged problems and situations calmly and objectively.

The salary range for this job opening at PeaceHealth is $246,600 – $343,200 . The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, seniority, etc.

PeaceHealth requires a completed Primary Vaccine Series (e.g., 2 dose monovalent Pfizer, Moderna or Novavax series or 1 dose J&J vaccine series) or be Up to Date (receiving the most recent Pfizer/Moderna bivalent vaccine/booster) for COVID-19 vaccination prior to their start date. PeaceHealth has a medical and religious exemption request process for those that are unable to receive the COVID-19 primary vaccine series due to medical/religious reasons. For caregivers that will be working in Oregon there is a personal vaccine exemption form that may be requested.
