Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Responsibilities The duties of this position include but are not limited to the following: The VA Portland Health Care System (VAPORHCS) is a Complexity Level la Facility and provides extensive primary, secondary, tertiary, and quaternary care in outpatient, inpatient, residential, and community settings. V APORHCS is a multiĀ­site health care system that includes a major medical facility in Portland, Oregon, outpatient and residential facilities in Vancouver, Washington, and 10 Community Based Outpatient Clinics. V APORHCS is an academic medical facility with significant teaching and research programs. The HUD-V ASH Licensed Professional Mental Health Counselor (LPMHC) is assigned to a HUD-V ASH team within Community Reintegration Service (CRS) under the Mental Health Division. The HUD-V ASH program utilizes a Housing First evidence-based clinical practice that centers on rapid housing for homeless people with high service needs, and then provides intensive case management services to obtain and sustain housing. The HUD-V ASH program serves a large catchment area and provides community-based services to Veterans who struggle with housing and employment stability related to complex socioeconomic factors that include psychiatric and medical diagnosis, substance use disorders, and involvement with justice system. LPMHC assigned to the HUD-V ASH program will be part of a multidisciplinary team. Much of the direct care will be provided in community settings and the LPMHC will be required to drive a government vehicle as they work with Veterans, and as they travel between sites of care. Incumbent will provide complete biopsychosocial assessments, assist Veterans with measurable, goal-oriented, treatment plans and be responsible for following VHA Directive 1162.05(1); Housing and Urban Development Department of Veterans Affairs Supportive Housing Program. The incumbent will provide individual, group and family counseling and interventions to assist Veterans with housing and employment stability and community reintegration. Case management resource brokering and documenting work in electronic medical record may be done in the community or office settings. Contact with Veterans will be in person, over the phone, and using video conferencing technology. Screening/ Assessment - The Licensed Professional Counselor will develop an assessment of the Veteran that meets both Joint Commission and CARF standards. The goal of the assessment is to highlight the Veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the Veteran's functional status. The biopsychosocial assessment will include clinical interview, administration of psychiatric test instruments, and/or collateral information. Based on the biopsychosocial assessment, the mental health Licensed Professional Counselor uses professional judgment and beginning practice skills to propose a multiaxial diagnosis assessment using the Diagnostic and Statistical Manual of Mental Disorders. The Licensed Professional Counselor provides goal planning, evaluation, and intervention with serious and complicated, highly diverse cases involving psychiatric illness. The Licensed Professional Counselor demonstrates an understanding of current techniques to the treatment of this Veteran population related to their physical limitations, psychological needs, psychiatric conditions, environmental needs and age associated illnesses and conditions. Treatment Planning/Goal Setting- The Licensed Professional Counselor develops and/or contributes to the development of the mental health treatment plan and the setting of achievable treatment goals with the Veteran/family and with the interdisciplinary treatment team. The Licensed Professional Counselor provides consultation and makes recommendations to the interdisciplinary team on course of treatment. Documentation/ Productivity - The Licensed Professional Counselor will complete documentation in compliance with appropriate documentation standards. The Licensed Professional Counselor consistently maintains an acceptable level of productivity per guidelines which may include special assignments. The Licensed Professional Counselor understands the challenges of bureaucracy and will act as an advocate when it serves the best interest of the Veteran/family. When appropriate and feasible, the Licensed Professional Counselor will educate and encourage the Veteran/family to advocate on own behalf, thus fostering a sense of independence and empowerment. Qualifications Basic Requirements: Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy). English Language Proficiency. LPMHCs must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3.j. Education. Hold a master's or doctoral degree in: Clinical Mental Health Counseling; Clinical Rehabilitation Counseling; Clinical Mental Health Counseling and Clinical Rehabilitation Counseling; or a related field, from a program accredited by the Council on Accreditation of Counseling and Related Educational Programs (CACREP). Examples of related mental health counseling fields include, but are not limited to Addiction Counseling; Community Counseling; Gerontology Counseling; Marital, Couple, and Family Counseling. TRANSCRIPTS WITH CONFER DATE REQUIRED NOTE: Traditional Rehabilitation counseling programs that are accredited by CACREP do not meet the LPMHC qualification standards as Traditional Rehabilitation counseling differs from Clinical Rehabilitation counseling. Licensure. Persons hired or reassigned to LPMHC positions in the GS-0183 series in VHA must hold a full, current, and unrestricted license to independently practice as a Licensed Professional Mental Health Counselor, which includes diagnosis and treatment. (Copy required for GS-11) Exception. VHA may waive the licensure requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure. This exception only applies up to the full performance level. For grade levels at or above the full performance level, the candidate must be licensed. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Licensed Professional Mental Health Counselor, GS-9 (Entry Level) Experience, Education, and Licensure. The GS-9 grade level is the entry grade level for LPMHCs with less than one year of experience(post-master's degree) or for LPMHCs at the master's or doctoral level) who are graduates not yet licensed at the independent practice level. Unlicensed LPMHCs at the GS-9 grade level have completed the required education listed in paragraph 3.b., and are working toward completion of prerequisites for licensure. In addition, the candidates must demonstrate the Knowledge, Skill and Abilities (KSAs) below: 1. Ability to engage in intake and assessment, problem identification, diagnosis, treatment planning and implementation, continuum of care, evaluation and follow-up. 2. Knowledge of current Diagnostic and Statistical Manual (DSM) of Mental Disorders in formulation of treatment goals and application of appropriate clinical interventions using professional counseling practices. 3. Basic knowledge of professional counseling practice with emphasis on human development principles through cognitive, affective, behavioral or systemic interventions, and strategies that address high risk behaviors, wellness, personal growth, and career development across the life span. 4. Ability to use a wide variety of individual, group, or family counseling interventions; demonstrates sensitivity to diversity and possesses multicultural counseling skills. 5. Ability to serve on an interdisciplinary team with focus on the intrapersonal, interpersonal, and relational functioning of individuals, groups and families and on the counseling approaches, strategies, and techniques that will best help patients function effectively. 6. Basic skill in the use of computer software applications for drafting documents, data management, maintaining accurate, timely and thorough clinical documentation and delivery of services, such as Telehealth. 7. Ability to learn and utilize software programs in use by VHA. 8. Knowledge and understanding of existing relevant statutes, case laws, ethical codes, and regulations affecting professional practice of counseling. 9. Ability to communicate effectively, both orally and in writing, with people from varied backgrounds. Licensed Professional Mental Health Counselor, GS-11 (Full Performance Level) Experience, Education, and Licensure. In addition to the basic requirements, candidates must have at least one year of post-master's or post- doctoral degree mental health counseling experience (VA or non-VA experience) and must be licensed to practice at the independent practice level and demonstrate the KSAs in subparagraph (c) below. LPMHCs may provide clinical supervision over the clinical practice of an unlicensed LPMHC or LPMHC trainee. Some state licenses do not license an individual to provide clinical supervision to trainees or unlicensed LPMHCs. Please refer to specific state licensing laws regarding the requirements for providing clinical supervision. In addition to meeting the Demonstrated Knowledge, Skills, and Abilities (KSAs) for the GS-9 grade level, the candidate must demonstrate all of the KSAs below: 1. Ability to make professional evaluations, decisions, and recommendation for treatment planning and implementation. 2. Advanced knowledge and mastery of the fundamentals of the counseling process which includes defining patient/family problems and maintaining an effective counseling relationship. 3. Ability to provide subject matter consultation to colleagues and trainees on the counseling process within various specialty areas, build on the foundation of competence through regular meetings and discussions to explain assignments, review progress of cases and confer about the counseling perspectives and orientation. 4. Ability to provide complex crisis intervention and stabilization to patients who are in psychological distress. Requires independent judgment and skill. 5. Ability to establish goals/treatment through a collaborative process with the patient utilizing advanced counseling skills, including evidenced- based practices, screening, and psychosocial assessment. 6. Ability to use a wide variety of individual, group, or familial counseling interventions; demonstrates sensitivity to diversity and possesses multicultural counseling skills. 7. Ability to fully utilize the current DSM in making diagnoses and formulation of treatment goals and application of appropriate clinical intervention using professional counseling practices. 8. Ability to develop and facilitate psychotherapy and psycho- education groups that include life skills, family support, and community integration. This may include evidence based psychotherapy. The full performance level of this vacancy is GS-11. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-9 to GS-11.
