Hourly Rate: $31.63

Position Summary:

Full time LPN position available for our Behavioral Health Program in Vancouver, WA. This position is part of the

Community Support Team, a unique community-based team that is built around providing the services that best fit each client's current needs. Travel throughout Clark County is required, must have a valid driver's license. The LPN is responsible for providing general nursing care to all patients as ordered by the patient’s provider and/or those procedures covered by current standing orders. The LPN may assist in the management of patient flow issues, patient acute illness screening, rooming patients, assisting providers in procedures and phone messages.

Duties and Responsibilities:

  • Demonstrates knowledge and skills necessary to provide care appropriate to the age of the

patients served.

  • Maintains professional standards and follows the treatment policies and procedures of the

organization.

  • The LPN must be familiar with sterile techniques, handling infectious cases, oxygen equipment,

and able to handle minor emergencies.

  • The LPN is responsible for leading, collecting and performing Clinical Quality Measures for

every patient; this includes patients being seen in the clinic as well as outreach to patients who

have yet to be seen.

  • Demonstrates knowledge, proficiency and compliance with regulatory requirements including,

but not limited to: Joint Commission, NPSG, Department of Health, Quality Measures, PCMH,

QI and other contractual obligations.

  • The LPN is the person who is responsible for ensuring that ALL patient clinical measures are

being addressed and will do so by doing thorough EHR reviews, tracking and documenting all

clinical measures/outcomes for patients in EHR.

  • Able to identify and categorize each patient’s age-specific grouping of needs, such as those for

infant, adolescent, or geriatric patients.

  • Prepares exam and procedure rooms by stocking with the appropriate supplies and disinfecting

as per established protocol.

Other duties may be assigned as necessary.

Qualifications and or Experience: 

  • Must have graduated from an accredited college or university and be licensed in the State of Washington to practice as a licensed practical nurse.

  • Must be qualified to administer injections/medications.

  • Must have Basic Life Support (BLS) CPR within 90 days of hire date and maintain this throughout employment.

  • Must have completed state mandated HIV education.

  • Excellent customer service skills.
