HIV Program Coordinator - Posting #26711

Hourly Rate: $30.00 - 31.50

Position Summary:

Full-time HIV Program Coordinator position available for our Vancouver Salmon Creek Medical Clinic in Vancouver, WA. The HIV Program Coordinator is a healthcare professional who works closely with healthcare providers and patients with measures and projects related to HIV. This includes screening, detection, prevention, linkage to care, and case management. The HIV Program Coordinator role has both patient-facing and administrative duties.

Responsibilities:

  • Screening and Detection of HIV

  • Works with providers to ensure recommended testing

  • Runs quality metric reports

  • Support of providers in screening and rescreening of higher risk patients

  • Works with the communications team regarding advertisement and outreach

  • Works with providers/clinical teams on support and training for HIV screening

  • Prevention of HIV

  • Case management of patients using HIV Pre-exposure Prophylaxis (PrEP)

  • Connecting patients with financial resources, including assisting with applications for programs such as PrEPDAP, Ready Set PrEP, and Gilead Advancing Access

  • Works with the HIV Clinical Champion to build, pilot, and run telePrEP programs and home-testing programs

  • Run quality metric reports

  • Works with the communications team regarding advertisement and outreach.

  • Works with providers/clinical teams on support and training for HIV PrEP.

  • HIV Coordination

  • When a patient tests positive for HIV, facilitate linkage to HIV ART prescriber within 30 days (14 days goal)

  • Connecting patients with financial resources, including assisting with applications for programs such as EIP/ADAP, and Gildead Advancing Access.  This may include urgent/ same-day processing with Prescription Assistance Programs to facilitate urgent ART in patients with newly diagnosed HIV.

  • Case managements of patients with HIV who are getting care at Sea Mar.

  • Work with providers/clinical teams on support and training for HIV diagnosis and linkage to care.

  • Role may include travel between clinic sites for associated duty, such as provider trainings, or patient-facing duties as above.

  • Works with the HIV Clinical Champion to build and sustain Sea Mar’s HIV program across the agency, and ensure consistency with other HIV initiatives in the organization.

  • Communicates with and builds partnerships with community organizations such as Gay City, Entre Hermanos, and POCAAN, etc.

  • Takes meeting minutes.

  • Other duties related to HIV screening, prevention, and coordination, as determined by the HIV Clinical Champion.

  • Other duties as assigned.

Qualifications:

  • Compassion and understanding for people living with a stigmatized chronic illness

  • Excellent verbal and written communication skills.

  • High level attention to detail required.

  • Must be able to respond quickly, efficiently and accurately to inquiries and requests.

  • Must strictly adhere to HIPAA compliance and other Sea Mar policies.

  • Must demonstrate problem-solving skills.

  • Cultural competence of the LGBTQ+ community.

Education and/or Experience:

  • Bachelor's Degree or equivalent experience required.
