Hourly Rate: $21.22

Position Summary:

Sea Mar Behavioral Health Department in Vancouver is seeking a full time Case Manager to join it's Program of Assertive Community Treatment (PACT). This position provides home-based counseling and case management services for mental health consumers that require non-conventional treatment modalities. Responsibilities may include client intake and assessment, treatment planning, clinical interventions (counseling, case management, crisis intervention), triage, consumer advocacy, coordination of care, clinical supervision. 

Duties and Responsibilities:

  • Participates in ongoing psychosocial assessment of the psychological, social, vocational, systemic and economic goals and needs of the mental health consumer. Ensures that treatment planning and interventions are client centered, culturally competent and age appropriate. Considers all aspects of the consumer’s, their families, and their community’s psychosocial, economic and cultural needs.

  • Provides clinical interventions and/or referrals to other providers to assist consumer in meeting

treatment and level of care needs. When providing direct treatment, develops and implements a

multidisciplinary treatment plan in collaboration with the consumer and others at the consumer’s

request. Clinical interventions may include: case management, system coordination, consumer advocacy, crisis management, supported housing, vocational rehabilitation, and supportive psychotherapy. Modalities such as individual, family, and group may be used.

  • Utilizes community and natural supports to supplement services, with the intent to better integrate the consumer into their community, and to eventually graduate the client from services when possible and clinically indicated. May help clients find jobs and/or volunteer opportunities.

  • Ensures that clinical interventions are effective, appropriate, and timely by maintaining a working

knowledge of psychiatric treatment and rehabilitation (including pharmacology), public and private

community resources, services and benefits, public income maintenance programs, federal and state laws, and agency policies and procedures.

Education and/or Experience:

  • Minimum of Associate's degree, Bachelor's degree in Social Work, Psychology or related field. 

  • Must have an active credential from the Washington State Department of Health (i.e. Agency Affiliated, LMHC, MHCA, LICSW, etc.). Must apply for Agency Affiliated Counselor credential and must obtain active AAC within 60 days of hiring date.

  • Must have a car, valid driver's license, current auto registration, and current auto insurance.

  • Candidate must demonstrate respect and appreciation for diversity and have an understanding of the community system.

  • Bilingual English/Spanish preferred not required
