Responsibilities

Hospital Medicine Perioperative and Recovery Excellence Team

Our Hospital Medicine team at Virginia Mason Franciscan Health, located in the beautiful Pacific Northwest, is looking for a motivated and enthusiastic physician to care for patients in our hospital at home program and our perioperative surgical home clinic.

You will benefit from being part of a small but mighty team focused on bringing hospital medicine expertise to patients in our hospital at home and perioperative environments. While serving on this team you will become an expert in perioperative medical care and telemedicine. You will develop an in-depth understanding of what acute care services can be provided in patient’s homes. You will offer these services for patients who would otherwise require overnight hospital stays.

You will benefit from the support of a dedicated medical director and clinical staff that support the program. As part of our multisite hospital medicine team, you will also have support and collaboration from a team of like-minded hospitalists.

Compensation Range: $306,000 to $340,000 annually depending on experience and performance with quality incentives.

Perioperative Surgical Home Program Overview

Program Summary:

The ideal candidate will be responsible for the preoperative risk assessment and optimization of patients undergoing surgery, ensuring they are in the best possible condition for their procedures. This role involves collaborating with a multidisciplinary team to deliver patient-centered care, enhance surgical outcomes, and improve overall patient experience.

Key Responsibilities:

  1. Preoperative Risk Assessment and Optimization:
  • Conduct comprehensive preoperative evaluations, including detailed medical histories, physical examinations, and risk assessments.

  • Order and interpret necessary diagnostic tests to optimize patients’ health status before surgery.

  • Develop and implement individualized preoperative care plans in collaboration with surgeons, anesthesiologists, and other healthcare providers.

  1. Risk Management:
  • Identify and manage any medical conditions or risk factors that could affect surgical outcomes.

  • Implement strategies to minimize surgical risks and complications, including optimizing patients’ chronic conditions and addressing any acute issues.

  • Provide recommendations for preoperative optimization, such as nutritional interventions, smoking cessation, and exercise regimens.

  1. Care Coordination:
  • Coordinate care with surgeons, anesthesiologists, nurses, and other healthcare professionals to ensure seamless preoperative processes.

  • Facilitate communication between patients, families, and the healthcare team to ensure all parties are well-informed.

  • Ensure that all necessary preoperative preparations, including medications, lifestyle modifications, and pre-surgical instructions, are completed and understood by patients.

  1. Quality Improvement:
  • Participate in quality improvement initiatives aimed at enhancing patient outcomes and program efficiency.

  • Collect and analyze data related to preoperative care processes and patient outcomes.

  • Implement evidence-based practices to improve the quality and safety of preoperative care.

  1. Professional Development:
  • Stay updated with the latest advancements in preoperative care, surgical practices, and risk management.

  • Participate in continuing medical education and professional development activities.

  • Mentor and educate medical students, residents, and other healthcare providers as needed.

Home Recovery Care Program Overview

Program Summary:

The ideal candidate will provide acute medical care to patients in their homes, ensuring they receive high-quality, hospital-level care in a familiar environment. This role involves close collaboration with a multidisciplinary team to manage a variety of acute and chronic conditions, improve patient outcomes, and enhance patient satisfaction.

Key Responsibilities:

  1. Patient Assessment and Care:
  • Conduct thorough initial assessments of patients referred to the Hospital at Home Care Program.

  • Develop and implement individualized care plans based on patient needs, medical history, and current condition.

  • Provide acute care management for a range of conditions that typically require hospitalization.

  • Monitor patients’ progress through daily telehealth consultations and follow up visits.

  • Adjust care plans as needed based on patient response and changes in condition.

  1. Care Coordination:
  • Collaborate with a multidisciplinary team, including nurses, physical therapists, social workers, and other healthcare professionals, to deliver coordinated and comprehensive care.

  • Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to care plans.

  • Coordinate care transitions, including hospital admissions, discharges, and handoffs to outpatient providers to ensure continuity of care.

  1. Patient Education and Support:
  • Educate patients and their families about their conditions, treatment plans, and self-care strategies.

  • Provide support and resources to help patients manage their health and prevent complications.

  • Address any concerns or questions from patients and families promptly and compassionately.

  1. Professional Development:
  • Stay updated with the latest advancements in home-based healthcare and hospital-at-home models.

  • Participate in continuing medical education and professional development activities.

  • Mentor and educate medical students, residents, and other healthcare providers as needed.

Qualifications

Requirements include:

· Medical Degree

· Completed ACGME Residency Training

· Board Certification - American Board of Internal Medicine

· ACLS, Washington State Medical License and DEA in good standing

Pay Range

$147.12 - $161.83 /hour
