PNW PACE employees provide team-based care and serve our participants at the PACE Center and in their homes. Position Summary The Home Care Coordinator - Non-Clinical is a unique opportunity for you to play a vital role providing administrative and operational support for the PACE program. You will work closely with leadership and an interdisciplinary team to coordinate homecare services ensuring they are provided in alignment with the Plan of Care in a participant-centric manner based on best practices and with a strong customer service focus. The Home Care Coordinator maintains oversight over the quality of services received and personal care service delivery through routine visits. Responsibilities You will coordinate and effectuate homecare authorizations for participants in the PACE program You will perform a wide range of administrative and operational duties such as scheduling visits, tracking results, documenting outcomes, producing reports and arranging/ordering supplies and durable medical equipment You will conduct assessments of homecare and home safety environments and document results You will ensure the plan of care and IDT recommendations are executed and home care workers are providing appropriate care to participants You will conduct routine visits to access contractor performance and communicate concerns and adherence to expectations Requirements Two years of experience performing any combination of administrative, scheduling, homecare, or patient care coordination duties Proficiency in Microsoft Word and Excel One year experience with the frail elderly/geriatric preferred Current CPR or obtained prior to position start Current Washington State Driver's License: and proof of a safe driving record (which meets established MHS Our Values As a MultiCare employee, we'll rely on you to reflect our core values of Respect, Integrity, Stewardship, Excellence, Collaboration and Kindness.
