Catholic Community Services, Family Behavioral Health is looking for a Patient Care Coordinator to join our team of innovators who continually "push the envelope" and challenges traditional thinking in Behavioral Health.

    The CCS Health Home team provides coordination of care directly with children and youth diagnosed with severe and chronic medical conditions in order to increase access to appropriate behavioral and physical health services, reduce emergency room visits and prevent medical crisis whenever possible. This position works flexible schedules in the home and community setting of the youth and families to ensure completion of necessary medical and mental health assessments with entry of medical documentation into an electronic health record, assists children and families in the identification of individual medical goals, teams with medical professionals and specialists to support a coordinated plan of care, and works to ensure the success of that plan.

Responsibilities

JOB SUMMARY:

  • Provide direct services to assess child for medical, mental health and chemical dependency risk factors, assess health status and demonstrate knowledge of health care needs and prescribed medications. Conduct a comprehensive health screening and history, including physical, mental and chemical dependency assessment and completion of clinical questionnaires.

  • Work in tandem with a child’s medical, behavioral, and specialty providers to ensure health care services are coordinated and addressing co-occurring conditions.

  • Take a lead role in developing and facilitating a clinical health team to address the child’s medical, clinical and social service needs. Team members may include physicians, occupational and physical therapists, RN’s and ARNP’s, mental health and chemical dependency providers, family members, housing supports and other professional and natural supports.

  • Assist child patient in collaboration with their family, in developing a Health Action Plan (HAP). Incorporate child’s support system, health care needs and providers and treatment goals and strategies updating the plan every 90 days.

  • Provide clinical supports to the child and their family to assist them in implementing the plan. Clinical support Includes treatment goals, strategies to overcome barriers, and actively assisting child and their family with connections and access to their services.

  • Provide direct services to the child and their family, facilitating transition home after their admission to a hospital or residential treatment facility. Includes planning for patient education that supports discharge care needs, medication management, follow-up appointments, and supported self-management of chronic or acute conditions.

  • Assist children, youth and their families in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance) identifying and utilizing available cultural resources whenever possible.

  • Document all interactions with the child and others involved in their health care timely according to contractual agreement and into an electronic health record.

  • Navigate and manage multiple confidential health care information systems to include; Insignia, Pre-Manage, Prism and Care Manager. This requires navigation of various medical based sites used to gather and assess risk and needs.

  • Track and organize child and family contacts for a large volume of referrals and enrolled families utilizing the tools available in the electronic health record.

  • This position will be based out of the Tacoma, WA site.

  • Other duties as assigned.

Qualifications

WHAT WE ARE SEEKING IN A CANDIDATE

  • Bachelor's degree in a related medical or behavioral health field with two years’ of experience.

  • Experience in working with individuals with co-morbid chronic condition management as well as behavioral change methods.

  • Ability to work both independently and as a member of a team.

  • Demonstrate the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations.

  • Ability to drive to our family homes and within the community with reliable transportation, valid driver’s license, and current automobile insurance with an acceptable driving record per CCS policy.

  • Ability to visually assess safety and risk within family homes.

  • Availability to flex some work hours, as needed.

  • Applicant must successfully pass required background clearances prior to an offer of employment.
