Washington State DSHS ALTSA Social Service Specialist 3 in Tacoma, Washington

As a Social Service Specialist with Home and Community Services (HCS), you will develop plans that provide the support necessary to allow clients to return to their community. Some of what you'll do: * Conduct and complete face-to-face comprehensive assessments in inpatient psychiatric settings to determine the level of care needed * Develop initial and ongoing care plans for clients that provide the support necessary to allow clients to return to their community * Provide case management to adults meeting criteria as set forth in agency policy, including authorizing payment and completing documentation * Provide case management to adults at high risk of re-institutionalization * Make appropriate referrals to community resources * Provide client advocacy, consultation, networking, family support, crisis interventions, and educate others about services provided What we're looking for: * Ability to complete assessments in inpatient psychiatric settings * Mental Health experience and education * Understanding of Long Term Care programs and policies * Ability to analyze a variety of programmatic issues/problems and develop workable solutions * Ability to identify and evaluate social dynamics affecting client choice and decision making * Knowledge of Medicaid terminology, regulations, and policies is a plus * Knowledge of social service payment systems and service authorizations procedures is a plus Professionals with one of the following should apply: * A Bachelor's degree in social or human services, behavioral sciences, criminal law or justice, or related field and 2 years experience equivalent to a Social Service Specialist 2 * A Master's degree in a related field AND one year experience equivalent to a Social Service Specialist 2 OR a Master's degree with a completed practicum * One year of experience as a Social Service Specialist 2.
