Summary:Provide Supportive Services to Veterans and their families in the Supportive Services to Veterans Families (SSVF) grant funded program. Support and assist case management services to Veterans to develop and execute Individual Housing Plans that enhance housing security to eligible participants through direct and indirect service provision in Goodwill and partner agencies. Must pass background check.Education and/or Experience:Bachelor's degree (B. A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.Essential Duties and Responsibilities include the following:Effectively assist participants in achieving housing stability.Provide supportive services needed by the participant to achieve and maintain stable housing.Establish and maintain good working relationships with participants, referral agencies, and all team members.Follow up on participant progress, providing encouragement as needed.Ensure all paperwork and related activities are current, accurate, and in compliance. Includes case notes, service reports, and attendance logs.Enter required data into Support 2020 Participant Case Management database in a timely and accurate manner.Provide hands on training, direct supervision, and support to participants.Submit data for continuous improvement outcome reporting system, as requested.Attend meetings, conferences, training, and workshops, as approved or required by supervisor.Assist in developing natural supports for participants.Assist participants in achieving the highest level of independent functioning while receiving services. HR286ER0114 Page 1 of 4 Supersedes all dated prior to 01/14Work with multiple participant caseload.Monitor, evaluate and record training activities and program effectiveness.Offer specific training programs to help participants maintain or improve housing stability.Monitor and record participant progress to ensure goals and objectives are met.Provide services in accordance with Goodwill policies, CARF standards, and principles of safety.Model appropriate work behaviors and interpersonal skills.Provide training using multiple methods, to reach all learning styles.Participate in meetings, staffings, and other activities as directed to promote and review the effectiveness of Individual Housing Plans and overall case management efforts.Follow all policies, procedures, and directives of Goodwill Industries assuring safety of personnel or property.Work collaboratively with Workforce & Family Services to provide client services.Properly wear and maintain all required Personal Protective Equipment (PPE).Maintain agency confidentiality.
