Job Summary

This position is responsible for the facilitation of all aspects of our Staying Strong Aftercare Program at Rising Strong, in addition to helping support families in engaging in Foundational Community Supports (FCS) for on-going formal (housing & employment) supports both before and after families exit the program. This position includes program administration, data tracking and organizing program events & public-facing education to community partners.  Ability to flux schedule around weekend and after hours events required.

Job Duties/Responsibilities

Assist RS families in developing a clear list of the goals they wish to achieve upon moving out of Rising Strong.

Create a case management file (either hard copy or via Salesforce) for each RS alumni, documenting case management services received, progress toward goals, and all applicable ROI's and program compliance documentation.

Act as a liaison, as needed, with other community agencies to assist alumni in meeting their self-identified goals.

Organize and host bi-monthly alumni events, yearly Resource Fairs and other alumni-specific activities in partnership with WISe Program Manager.

  1. As applicable: coordinate transportation to and from events and all aspects of the event workflow.

Support & attend scheduled graduation & celebration events.

Facilitate survey administration/data collection for all alumni enrolled in the program to ensure qualitative data is collected & captured, aligning with program goals.

Provide feedback, as needed, to assist alumni with problem-solving, conflict resolution, parenting and interpersonal concerns and attainment of goals.

Oversee training of alumni in budgeting, rental rights and responsibilities and community awareness skills.

Provide access to healthcare resources to maintain mental and physical healthcare for alumni families.

Assist families with maintaining housing/voucher requirements and accessing employment resources.

Conduct home visits for on-going support, where applicable.
