Director of Case Management

SUMMARY STATEMENT & PURPOSE Under the general direction of the Director of Case Management, the Social Worker completes the process of planning, organizing, coordinating, and monitoring the services and resources needed for individual healthcare needs. Social Work serves in a professional role to coordinate the patient’s care in collaboration with the interdisciplinary treatment team which supports the delivery of quality care in the most cost effective way by managing the human, material, and financial resources available for the patient’s care. All employees are expected to be pleasant, respectful and courteous in all interactions with patients, families, staff, and visitors of the Institute, as well as being a positive representative of the hospital at all times.

MINIMUM QUALIFICATIONS

  • Masters in Science with emphasis on sociology or psychology

  • Must have sufficient training and experience in psychosocial aspects of physical disabilities and illness.

PREFERRED QUALIFICATIONS

  • Master's Degree in Social Work with medical experience
