Providence St. Joseph Health Social Worker MSW - PACE/Elderplace in Spokane, Washington
Description $3,000 hiring bonus for eligible external candidates that meet required qualifications and conditions for payment. As a member of the interdisciplinary team, provides social work services to participants in a manner which is consistent with the mission and core values of Providence Health System. Functions as primary outreach, education and marketing worker. Fields inquiry calls. Schedules and completes facility tours to individuals and groups. Assists potential participants through the application process, completes intakes, initial assessment, quarterly assessment, annual assessment, and case management services. Provides direct support and supervision to program participants. Makes appropriate external referrals as needed. Effectively communicates with community stakeholders to provide excellent continuity of care to each participant. Required qualifications: Master's Degree in MSW from an accredited school of social work Or equivalent educ/experience 2 years Social work (or other human services) experience including effective work with geriatric age groups from diverse ethnic cultures. Required upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving.
