This position is responsible for working one-on-one with at-risk students and their families with the goal of improving the family’s housing stability, financial stability, educational attainment, and access to resources.Job Duties/ResponsibilitiesTo perform this job successfully, an individual must be able to satisfactorily perform each of the essential duties/responsibilities listed below. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential duties/responsibilities of this job.This job may include any of the responsibilities listed below:Complete intakes and assessments of all referred families to determine their qualification and need for the program.Work with students and their families to assess their barriers and connect them to financial, housing, educational and other community resources to help reduce or alleviate barriers.Assist clients in developing an individualizedclient-drivencase plan that will enable them to obtain needed resources for ongoing stability.Create a case management file for each family, documenting case management services received and progress toward goals.Monitor student attendance, behaviors, and grades, while working with the school and community partners to evaluate academic success.Meet with clients either within their home, school, or agreed upon community center (where applicable) to review progress toward goals and identify current action steps needed to achieve goals.Act as landlord liaison. Coordinate with landlords prior to and during clients’ tenancy to assure that tenancy issues are addressed pro-actively.Act as an advocate with other community agencies.When applicable, ensure clients continue to meet grant requirements for financial assistance.Provide referrals and information related to problem-solving, conflict resolution, parenting and interpersonal concern in order to assist clients in achieving personal goalsEnter data into established entry system(s), exit and follow up, as required; provide monthly reports documenting activities for funding sources and attend required meeting as applicable.Coordinate with other SMS staff, school districts, and funders to develop and oversee activities that will benefit the entire program.Adhere to the tenets of Catholic Social Teaching and Catholic Doctrine.Perform related functions necessary to support the mission and core values of Catholic Charities.Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI), per regulations outlined i
