General Dynamics Information Technology Case Manager – Medical Services in Spokane, Washington

The Case Manager supports the Medical Services Team within a government agency, in planning and coordinating services to meet the health needs of individual program participants, and to promote quality and cost-effective outcomes in the participant’s best interest while in care and upon release.

Duties include but not limited to:

  • Work closely with the Medical Services Team on identifying and tracking individuals with complex medical and mental health conditions and individuals that require health care services/treatment while in care and/or post release

  • Collaborate with care provider staff to include clinical, medical and case management staff involved in individual cases to assess and plan for treatment and discharge planning

  • Establish and maintain relationships with federal, state, local, and non-profit health agencies to coordinate the transition of medical services for individuals while in care and upon release

  • Create and sustain partnerships with community resources and support agencies, as well as coordinating with post release services

  • Maintain a database of charitable and pro-bono services for program participants upon release from care

  • Maintain a database of national health insurance programs that provide coverage for uninsured/ineligible program participants

  • Support a quality assurance/process improvement program for the health program

  • Participate in medical site visits and needs assessments, as needed

  • Train and provide technical consultation to government staff, grantees, and other stakeholders on medical case management on a variety of topics, such as:

  • Performing assessments of the physical, functional, social, psychological, environmental, learning and financial needs of patients that are elevated to headquarters for review

  • Identifying problems, goals and interventions designed to meet patient’s needs

  • Formulating care plans

  • Providing appropriate interventions which demonstrate knowledge of and sensitivity toward cultural diversity

  • Assessing the patient’s formal and informal support systems

  • Implementing and monitoring the care plan to ensure the effectiveness and appropriateness of services

  • Evaluating patient’s progress toward goal achievement

  • Researching alternative treatment options

  • Acting as a patient advocate and assist with problem solving

  • Providing referrals to appropriate community resources

Qualifications

  • 5-8 years of relevant experience

  • Strong knowledge of pediatric health issues

  • Strong analytic capabilities and writing skills

  • Strong organization and communication skills

  • Background in program case management, quality assurance or continuous program improvement, data analytics, research methodology

  • Qualifications of flexibility, collegiality, creativity, and motivation

  • Commitment to serving at-risk individuals and communities

  • Experience or expertise in immigrant health

  • Proven track record of program management

  • A minimum of bachelor’s level training in nursing (BSN) or clinical social work

  • Case Management certification

  • Master’s level Social work degree (LCSW) or Master’s degree in Public Health preferred.
