Responsible for social work evaluation of patient/family psych/social and environmental needs, as well as implementation of the social work treatment plan and documentation consistent with insurance guidelines. Upon hire: Washington Advanced Social Worker License Or Upon hire: Washington Advanced Social Worker Associate License Or Upon hire: Washington Clinical Independent Social Worker License Or Upon hire: Washington Clinical Independent Social Worker Associate License 1 year Experience in a health care setting. 3 years Post Master's work experience, or prior experience in hospice care setting. About the Team Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint. HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses.
