UW MEDICAL CENTER - Northwest SOCIAL WORK has an outstanding opportunity for a FULL-TIME SOCIAL WORKER SUPERVISOR, DAY SHIFTS . The Social Worker Supervisor serves in a supervisory role, providing clinical supervision and consultation to social work staff and students, and assisting in departmental administrative activities. This clinician must maintain current knowledge of psychosocial factors impacting healthcare, available resources to address psychosocial needs of patients, barriers to discharge, and eligibility requirements, referral processes and regulations for community services and/or post-acute care. This position collaborates with agencies/providers/payors that currently are engaged with UWMC to ensure coordination of care. RESPONSIBILITIES: • Provide supervision to assigned supervisees and students, conducts performance evaluations, and initiates performance improvement activities when appropriate • Supports patient care optimizing patient safety, treats patients and their families with respect and dignity, involving them in their care whenever possible. • Provides counseling and support during emergent issues and/or changes in status that may impact patient’s outcome • Provides assistance in situations requiring identification of LNOK, advance directives, and facilitation of guardianships • Demonstrates a commitment to advancing equity, diversity and inclusion within the department and organization. • Assists in working with Social Work students through involvement with the Practicum program and/or mentoring individual students • Assists the department management team on key initiatives, employee engagement and rounding, and formal and informal training on social work-specific issues • Serves as Weekend AOC on a rotating basis. • Provides assistance in on-boarding new social work staff • Provides individual and group clinical supervision • Collaborates with agencies/providers/payors that currently are engaged with UWMC patients, to ensure coordination of care • Assists the department with clinical or problem-solving consultation requests from all department staff and clinics • Assists the management team with arranging for coverage for planned and unplanned absences • Act as point person for remote, on call support to assigned outpatient clinics at Northwest and other duties as assigned
