Responsibilities

The Social Worker provides social work interventions for priority Medical Center patient and family needs.

  • Completes behavioral health and psychosocial assessments

  • Connects patients with resources appropriate to meet transition planning needs

  • Communicates effectively with outside facilities and agencies, coordinating patient needs

  • Provides short term counseling in an effort to support paitents and families through hospitalization and illness

  • Coordinates a multidisciplinary plan of care with the inpatient/outpatient care team

  • Completes documentation per departmental policy

  • Works within regulatory requirements from CMS, The Joint Commission, and other regulatory agencies. Works within legal, regulatory, accreditation and ethical practice standards relevant to the position and as established by Virginia Mason; follows safe practices required for the position; complies with appropriate VM policies and procedures; fulfills any training required by VM, as appropriate; brings potential matters of non-compliance to the attention of the supervisor or other appropriate hospital/clinical staff.

  • Provide social work services to patients and their families, including psychosocial assessment, domestic violence screening, child abuse and elder abuse interventions, and discharge planning for inpatients admitted to the assigned medical or surgical service

  • Screening and intervention domestic violence, for abuse and neglect, and substance abuse

  • Other duties as assigned

Qualifications

  • Master's of Social Work from an accredited college or university and Licensed Independent Clinical Social Work (LICSW) licenseĀ· 2 years of Social Work experience in an inpatient setting

  • Strong interpersonal skills; a working knowledge of individual and family dynamics; strong organizational skills, ability to multitask, an understanding of systems and how to work within systems; an understanding of the impact and implications of disease and the course of disease; excellent communication skills; the ability to perform psycho/social assessments; and a working knowledge of community resources.

Prefer three years in a care coordination, caee management or discharge planning role

Pay Range

$38.64 - $56.03 /hour
