$10,000 Signing Bonus to eligible new employees joining UW Medicine University of Washington Medical Center - Montlake Heart Institute has an outstanding opportunity for a Registered Nurse Post Heart Transplant Coordinator to join our Cardiac Transplant team. DEPARTMENT DESCRIPTION: UW Medicine's Heart Institute is one of only eight elite facilities in the nation to achieve the highest possible score from the Scientific Registry of Transplant Recipients. PRIMARY JOB RESPONSIBILITIES: • Provide expert nursing to post heart transplant patients • Systematically assess the need for and the delivery of nursing services that meet patient care requirements, whether delivered directly or by the other nursing staff • Ability to formulate, coordinate and revise an individualized plan of care as indicated by the patient’s response to treatment and evaluate overall plan daily for effectiveness • Provide care for individual patients over the telephone using the nursing process • Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors • Integrate patient and family education into the delivery of care, ensuring educational activities are culturally appropriate and are tailored to the patient’s and family’s ability to comprehend and participate in the post-heart care • Coordinates the care given by others to achieve efficient clinic operations • Assume responsibility for continued professional growth and sharing expertise with other team members

REQUIREMENTS: • Active WA State or Multi-state RN License by start date • Current BLS and ACLS • 2 years of outpatient or inpatient Cardiology/Tele experience
