UW Medical Center-Montlake - Transplant Post Liver has an outstanding opportunity to hire a PROGRAM COORDINATOR / UW MEDICINE PATIENT SCHEDULER .

WORK SCHEDULE

  • 40 hours per week, Day Shift POSITION HIGHLIGHTS

  • The Program Coordinator will serve under general directions of a multidisciplinary team, and perform work using knowledge and experience specific to the Liver Care and Transplantation service line, specifically the post liver transplant program

  • Be a part of a team dedicated to supporting UW Medicine where Patients Are First PRIMARY JOB RESPONSIBILITIES

  • Coordinate and schedule diagnostic tests, infusions, and consultative appointments necessary for post liver transplant care, including independently prioritizing the needs of program clients; specifically coordinate post liver transplant ambulatory (outpatient) continuum of care for patients 90 days post liver transplant throughout the rest of their lives

  • Manage patient clinical information and files; coordinate the timely exchange or receipt of program requirements such as patient forms, medical records, outgoing referrals, and lab and diagnostic results from UWMC, primary care physicians’ and gastroenterologists’ offices and other medical facilities

  • Interact with providers, nurses and other team members to schedule all diagnostic tests as ordered by providers

  • Coordinate multiple types of infusions for patients experiencing organ rejection or other complications, e.g. IVIG, including submission for financial clearance by the FACT team, and arranging admission to the hospital (4South)

  • Prepare schedules for diagnostic tests, infusions and clinic visits, plus interact with patient and family/caregiver to provide necessary program instructions and to discuss itinerary of the diagnostic tests, consultative appointments, pre-test patient preparation instructions, and answer questions

  • Maintain documentation for patient visits, including Telemedicine visits, and EPIC for Telemedicine visits

  • Participate in the preparation and maintenance of data

  • Support liver transplant service activities

  • Participate in on-going process improvements, maintenance and development of routines, standards, and goals REQUIRED POSITION QUALIFICATIONS

  • High school graduation or equivalent AND two years of experience in a program specialty OR equivalent education/experience.
