The position acts as an innovator and integrator across the network, convening Coordinated Care Organization (CCO) partners to co-create, learn together, and spread emerging best practices for improving care for CareOregon populations. This highly creative leadership position is responsible for promoting a great degree of intra-departmental collaboration and integration. Functionally, this position oversees the regional population health and high-risk strategy. Essential Responsibilities Participate in the development, implementation, and evaluation of population health programs, including high risk CareOregon members in partnership with the CareOregon network and other community partners. Create, recommend, facilitate, and evaluate population health strategies and care enhancing cost-control programs that serve members across CareOregon's regional population. Serve as the content expert for Population Segmentation and other population health tools for the region. Promote collaboration with delivery system leaders across the region to build a strong foundation of population health program support. Develop and maintain strategic partnerships with community-based social service organizations in partnership with community health staff to assure CareOregon can leverage and improve access to upstream non-medical health-enhancing services for its members. Support collaborative relationships with providers that promote and achieve Triple Aim goals. Ensure strategic performance goals, expectations and standards are clearly understood by stakeholders. Evaluate the Population Health Portfolio performance on an ongoing basis and take appropriate corrective action. Represent CareOregon on appropriate local, state, and national committees and work groups. Strategically inform and provide guidance from a population health perspective to care management teams.
