The Care Management Department has an outstanding opportunity for a NURSE CARE COORDINATOR. The goal of the Nurse Care Coordinator is to ensure delivery of optimal care for the patient while promoting fiscal responsibility. SCHEDULE: 100%, days. RESPONSIBILITIES:

  • Screens all new admissions to identify potential discharge barriers and risks for readmission

  • Facilitates discharges to home, including arranging all in-home needs of equipment, services, and care-giving plans

  • Facilitates daily medical service interdisciplinary team huddles to help the team determine a “plan for the day” and “plan for the stay.”

  • Collaborates with the medical team to estimate the LOS and ensuring this information is communicated to the interdisciplinary team

  • Tracks data on discharge barriers, suggesting and participating in process improvements aimed at reducing LOS and readmission

  • Assists the team in identifying internal consult services that could advance the plan of care

  • Facilitates/participates in care conferences to troubleshoot barriers to discharge for medically or socially complex patients

  • Assists the team in identifying potential external resources that ensure a safe transition to the post-acute care setting

  • Tracks patient progress on care pathways and alerts and assisting the team when patient is off pathway

  • Facilitates/participates in unit-based interdisciplinary team huddles including the charge nurse, SW and Therapies to increase awareness about estimated LOS for individual patients and to help units prepare for morning discharges

  • Partners with the primary team to create care plans for patients with a history of, or risk for, readmission

  • Consulting with social work on complex medical needs affecting discharge REQUIREMENTS

  • BSN

  • 5 years in clinical practice in an acute care setting LEGAL REQUIREMENT
