Providence St. Joseph Health Navigator RN - Stroke Clinic in Seattle, Washington

Description The RN Stroke Navigator will contact the patient before hospital discharge,and then follow the patient after discharge providing coordination between the patient, PCP, neurologist, and other specialists. This position will perform in-hospital and early post-discharge screenings for medication compliance, healthy life style, depression, medical complications, and caregiver burden while being an advocate and anticipating their needs. This increased coordination will benefit patient outcomes and satisfaction, while decreasing unnecessary health care expenditures such as readmission. Required qualifications: Washington Registered Nurse License upon hire National Provider BLS - American Heart Association upon hire Modified Rankin certification within 30 days of hire 5 years related neurological clinical experience Preferred qualifications: Bachelor's Degree in Nursing Master's Degree in Nursing Stroke Certified Registered Nurse upon hire American Association of Neuroscience Nursing membership and certification upon hire 1 year experience in an Ambulatory setting Experience working with patients who have neurological conditions, and their families Experience working in a certified Comprehensive Stroke Center Experience leading formal educational forums/in-services Why Join Providence?
