Providence Lead Clinical Liaison RN in Seattle, Washington

This position serves as the Supervisor/Lead Clinical Liaison to the Hub Team(s) and provides leadership and coordination for the care provided to patients and their families referred to home health, hospice, and other post-acute care services that are offered with PHCC. In addition, this position supports, as directed, all Business Development leaders across the Director's full territory. This position is responsible for identifying 'best practices' for care delivery and may network with community resources and consult with case management at the hospital.

The Supervisor/Lead Clinical Liaison is a lead position and provides day to day oversight and support of the hub liaison team to include RN's and Care Transition Associates, in addition to working in the clinical liaison role. As a part of the leadership team, this position will play a decision maker role, specifically for referrals/patients needing review for approval.

The Lead Clinical Liaison is responsible for creating and maintaining positive customer relationships in order to increase referrals for Home Health, Hospice / Palliative Care, Transitional Care Units, and other services within assigned accounts. This role includes working with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services, and facilitate the coordination of services and to ensure there is appropriate documentation for compliance with agency and statutory guidelines to manage the transition of the patient from an acute care facility to the patient's home or next location of residence. Consultative functions include, but are not limited to: providing consultation and informational presentations regarding end of life care and home health; sharing expertise through in-services on pain and symptom management options; serving as a clinical resource and educator for physicians, facility staff, patients, families and community on Home Health, Hospice / Palliative Care or other issues in various settings and facilities. The Lead Clinical Liaison coordinates closely with Account Representative to build referral volumes in assigned accounts and develop opportunities for educational outreach that will enhance service awareness and access. Additional duties assigned may include collaboration with local Service Line leadership in essential meetings, reporting on tactical plans, training/orientation, and day-to-day support of the business development staff.

Required Qualifications:

  • Education sufficient for licensure

  • Bachelor's degree in Nursing, or a Diploma in Nursing or an Associate's Degree in Nursing (ADN)

  • Upon hire: California Registered Nurse License or

  • Upon hire: Washington Registered Nurse License

  • Upon hire: Unencumbered Registered Nurse License in state of hire and wherever care is delivered

  • Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information

  • 5 years experience in clinical discipline

Preferred Qualifications:

  • A minimum of three (3) years of staff supervision experience

  • Previous liaison, marketing, sales, or health care industry experience

  • 3 years home health, hospice and/or palliative care experience
