Swedish Health Services Inpatient Clinical Social Worker MSW - Ballard in Seattle, Washington

Swedish Medical Center is looking for a Social Work Case Mgr $5,000 Sign-On Bonus (1.0 FTE, Day Shift) to work in Case Management - Ballard at Swedish Ballard in Seattle, WA.

This position will work five, 8 hour shifts per week (40 hours; 8am-4:30pm PST). Days to be worked are Monday through Friday.

The Clinical Social Worker partners with patients, families and the health care team to address and advocate for patients' social and emotional needs. The clinical social worker is responsible for providing a full range of social work services including but not limited to psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. The clinical social worker partners with the patient and his or her support system, as well as interdisciplinary teams, both internal and external to the organization, to improve patient care through the effective utilization and monitoring of health care resources. The clinical social worker assumes a leadership role to facilitate the achievement of patient goals and desired clinical, financial, and resource outcomes. The clinical social worker is guided by the NASW Code of Ethics and the Swedish Mission and Vision in his or her daily practice.

In this position you will:

  • Collaboration: Work collaboratively with patients nurses, case managers, physicians, other practitioners, caregivers and community resources and agencies. The social worker and healthcare team are jointly accountable for measurable outcomes that are cost effective and reflect patient preferences and values.

  • Communication: Communicate timely, relevant and accurate information to all parties involved with a patient’s care.

  • Facilitation: Facilitate the progression of care by advancing the care plan to achieve desired outcomes.

  • Coordination: Integrate the work of the healthcare team by coordinating resources and services necessary to accomplish agreed-upon goals.

  • Advocacy: Advocate on behalf of patients and caregivers for service access or creation, and for the protection of the patient’s health, safety and rights.

Qualifications:

Required qualifications for this position include:

  • Master's Degree in Social Work from an accredited school

  • 1 year experience in the areas of acute medical or mental healthcare in an inpatient or outpatient setting

  • Licensed Social Work Associate – Independent Clinical (LSWAIC)

  • Compliance with the NASW Code of Ethics

Preferred qualifications for this position include:

  • Licensure as an Independent Clinical Social Worker (LICSW) with the Washington State Department of Health under Chapter 18.225 RCW is preferred and may be substituted

  • ACM/CCM certification
