Accolade is currently Recruiting for a Full Time Pediatric/Neonatal Intensive Care Unit (NICU) Complex Case Manager Registered Nurse. This position is Remote. Applicants will be required to work 8:00am to 4:30pm your local time. Role overview As an Accolade Pediatric/NICU Complex Case Manager Registered Nurse, you will play a crucial role as a trusted and reliable resource for our members requiring ongoing management of complex or chronic health conditions. Leveraging your clinical case management expertise and applying Accolade's core value of genuine care for members, telephonically you will conduct thorough assessments of members' health concerns, prioritize medical needs, build comprehensive care plans, and provide follow up guidance. A day in the life... Accolade RN Complex Case Managers are entrusted with providing information to our diverse population of members that empowers them in making informed health decisions. We partner with our members in various ways (via telephone and digital), and apply critical thinking measures to achieve the following: * Collaborate with members and their interdisciplinary care teams to develop comprehensive and effective personalized plans for care to support the members' healthcare journey. * Engage with members via inbound and outbound telephonic communication, providing education on Accolade's services and appropriate clinical program enrollment. * Conduct member behavioral and clinical assessments to identify an individual's holistic needs, understanding what's important to them and advocating for members to receive the right care at the right time * Effectively and confidently communicate comprehensive personalized care plan information, including but not limited to medical tests, procedures, costs, risks, treatment options, and care journey specifics, including preparation for hospital admission, inpatient stay and discharge * Professionally navigate members' health plans to facilitate coverage and care decisions by demonstrating an understanding of our members' employer-sponsored benefits * Guide members and member-appointed delegates to appropriate facilities, agencies, providers, and specialists for medical needs What we are looking for... Required: * Bachelor's degree in nursing (BSN) or equivalent * Compact licensure * Possess current/active Certified Case Manager License (CCM) or commitment to obtain certification within one year of hire * 5 years minimum as an active Registered Nurse with a current unrestricted license * Minimum of 3 years of experience as a Neonatal Intensive Care Unit (NICU) Registered Nurse. * 1 year minimum telephonic case management experience * Ability to navigate and work with various EMR technologies * Multitasking abilities to include active listening, typing, and member inte To view the full job description,
