Swedish Health Services Cultural Navigator Seattle, WA in Seattle, Washington


Swedish Medical Center is looking for a Community Cultural Navigator (1.0 FTE / 40 hours per week, Day Shift) to work in our community in Seattle, WA. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible to all.

The Cultural Navigator program is seeking bilingual and/or bicultural individuals who are committed, responsible, empathetic, and motivated to provide opportunities and build bridges for a stronger community.

Cultural Navigators are Community Healthcare Workers who have knowledge of the local community and resources and a deep understanding of their communities needs through lived experience. Community education, outreach, identification of barriers to care, referral to resources, and educating healthcare providers in culturally appropriate delivery of care are key activities. Cultural Navigators will work with patients to connect them to health and community services and act as advocates to promote culturally sensitive health care. In this position you will:


  • Assesses patient’s knowledge and ability to navigate community services

  • Assists patient in learning to navigate the healthcare system to overcome barriers to care

  • Identifies gaps in care and helps to facilitate access to needed services

  • Assists patient to complete medical, financial or other forms as needed

  • Build the capacity of clients/families to address health and social issues.


  • Advocate for culturally/linguistically appropriate services and support.

  • Educate/advocate clients/families on how to navigate the health care system.

  • Create connections between population served and health care systems.

  • Supports and advocates for patient and family expressing diverse perspectives and health care needs reflecting health equity

  • Promote health and wellness including mental, emotional, occupational and spiritual health

  • Provide education on disease prevention, self-management of chronic illnesses and medication adherence


  • Represents Swedish Medical Center in outreach and education activities and works to build relationships with community members and organizations

  • Participates in educational and outreach events and mobile clinic opportunities in response to health and social service needs

  • Participate in health promotion and outreach activities in the community, in conjunction with internal and external partners.

  • Will identify key community organizations to work with to coordinate outreach presentations, events and direct service opportunities

  • Advocates for clients in health care and social service arenas


  • Works as a team member with the other members of the Cultural Navigator Team, Interpreter Services and other clinic and hospital staff

  • Collaborates with diverse community organizations and members

  • Attends and participates in regular staff and department-wide meetings

  • Assist in the design, development and messaging of the Cultural Navigator Program to be disseminated to communities, including the development of communication tools and social networking.

  • Documents all contacts and pertinent information in the medical record in a timely manner and maintains work logs for data collection

At Swedish, we are passionate about healthcare for our caregivers. Full-time caregivers (0.75-1.0 FTE) who earn less than $60,000 per year will be offered the Swedish PPO Medical Plan with a $0 premium cost, including dependent coverage

Required qualifications for this position include:

  • H.S. Diploma or GED or equivalent educ/experience

  • 3 years of Bilingual. Demonstrated verbal and written fluency. Posting will be unique to specific language.

  • Prior to hire, bilingual applicants must complete a bilingual caregiver fluency evaluation.

  • Valid Washington State driver's license required

Preferred qualifications for this position include:

  • Associate's Degree in Health or Social Work Related Field

  • Certified Medical Interpreter for Spanish, Arabic, Mandarin/Cantonese, or Vietnamese

  • Certified Community Health Worker
