Providence St. Joseph Health Community Health Worker in Seattle, Washington
Description Communicates care coordination information by phone, fax and email in accordance with health system policy. Works directly with patients and their families to address social barriers to health. Ensures member access to services appropriate to their health needs. Conducts screenings and gathers information to identify individual needs for addition to the care management plan. Coordinates internal and external resources to meet identified needs. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
