Providence St. Joseph Health Care Integration Coordinator - PacMed Seattle - Accountable Care in Seattle, Washington

Description Supports the population health team by entering/coordinating incoming referrals and tracking outreach. Makes outgoing calls/sends Mychart messages to schedule appointments, provide resources, coordinate care, and provide class and visit preparation instructions to patients. Answers incoming phone calls from patients, providers, clinic staff and resources. Compiles data and prepares/submits reports for program, care and organizational performance metrics. Schedules conference rooms/venues for education classes. Provides routine financial counseling, researches routine referral and billing issues, and completes registration and data entry for programs. Responsible for set-up of all equipment, furniture, and other resources needed for classes. Takes and distributes departmental meeting minutes. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
