Job Summary

Directly supports the YMCA’s care coordination process of connecting individuals referred by their health care providers or through self-referral to the YMCA’s suite of chronic disease prevention and management programs through engaging in phone- and email-based outreach to these individuals and conducting motivational interviewing conversations with each person. Provides administrative, organizational, and logistical support to the Healthy Living Center of Excellence and assists in quality assurance and oversight of chronic disease prevention programs.

Hiring Range: $22 - $24.20/hour

Responsibilities

  • Maintains an electronic database of private health information and enters and sends/receives data through the referral management system following all privacy and security protocols.

  • Conducts care coordination conversations with referred individuals and other prospective community members utilizing motivational interviewing and active listening skills.

  • Assists with marketing and retention activities to create robust classes with high enrollment.

  • Ensures accurate intake forms, data collection forms and appropriate program documentation are completed.

  • Ensures that program/enrollment targets are met and consistent with program standards.

  • Supports quality assurance and other administrative duties related to chronic disease programming across the Association.

  • Other duties as assigned.

Qualifications

  • One or more years of education and/or work experience in health promotion, public health, community outreach, or similar field.

  • Working knowledge of preventative health programs and philosophy.

  • Comfort interacting with individuals’ health-related information and communicating with health care providers.

  • Intermediate to advanced computer skills and experience with Microsoft Office suite.

  • Ability to visit sites and community organizations

  • YMCA Healthy Lifestyles Principles certification*

  • Listen First Level I* and II** training

  • Ability to respond to safety and emergency situations.

  • Current state approved first aid certification.

  • Current state approved CPR for the Professional Rescuer certification.

    • Within 30 days of employment or first available training.

    ** Within 90 days of employment or first available training.

    PREFERRED EDUCATION AND EXPERIENCE

  • One or more years of college education in health promotion, public health, or a related field. Other applicable education, training, and experience, which provide the knowledge, abilities, and skills necessary to perform effectively in the position will be considered.

  • Two or more years’ experience working in health promotion, public health, or community outreach or similar field.

  • Prefer knowledge of and previous experience with, diverse populations (language, culture, race, physical ability, sexual orientation, etc.). Ability to speak any language in addition to English may be helpful.

  • Prefer any type of previous experience interacting with the healthcare system
