Licensed Practical Nurse - Posting #26301

Hourly Rate: $30.70

Position Summary:

Full-Time (40 hours per week) position available at Cannon House, our Assisted Living Community in Seattle. Currently looking for a dynamic, critical thinker with excellent clinical, and leadership skills and an interest in working with an ethnically diverse resident population.  The successful applicant will work with all members of the interdisciplinary care team and must have good interpersonal and communication skills (both written and verbal).

Under the direction of the Resident Care Manager and the Wellness Coordinator, the successful applicant is responsible for ongoing resident assessments, providing excellent nursing care to facility residents, insuring that their health care needs are met; ensuring timely documentation and meeting resident care and other performance responsibilities in a timely manner.  Successful applicant must be a proactive participant in care coordination and service delivery to facility residents.

Duties and Responsibilities:

  • Provide direct care and services to facility Residents as per organizational protocol, i.e. insulin monitoring and injections, etc.

  • Complete a monthly summary progress note in the resident's medical record after performing a head to toe assessment on the resident.

  • Participate in the annual and/or change of condition reassessment of residents as per Resident Care Manager and/or Wellness Coordinator request.

  • Insure timely nursing and provider-ordered interventions.

  • Work closely with the Resident, the Resident's provider and other members of the interdisciplinary team in coordinating care services.

Other duties may be assigned as necessary.

Qualifications and or Experience: 

  • Must have a WA State Active LPN License and have successfully completed the state mandated HIV education.

  • Must obtain continuing education credits to meet licensure requirements.

  • Experience with Long-Term Care or Geriatric Care a plus but not necessary.

  • Must have Basic Life Support (BLS) CPR within 90 days of hire date and maintain this throughout employment.

  • Must have a basic understanding of managed health care and the mechanics of DSHS
