Providence St. Joseph Health Patient Liaison - USFHP in Renton, Washington

Description (This position will be in person for the first 90 days. After 90 days, may be eligible to work from home, 1-2 times a week) Supports and works collaboratively with internal and external groups to support our commitment to high-quality, value-based care. The Population Health Coordinator operates in a highly autonomous manner, utilizes a diverse set of problem solving skills, and is able to navigate multiple data sources to obtain documentation to close gaps in care and improve quality metrics. Under the guidance of the Quality Manager and/or Quality Analyst, reviews quarterly performance reports, meets with network group representatives, and develops detailed action plans to ensure data is obtained to close gaps in care. This position provides support for HEDIS activities, HCC coding audits, TRICARE reports, quality measure reports for provider groups, and reports to support utilization and enrollment activities. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. 2 years Customer service related experience. Prior experience using Excel spreadsheets. Prior experience with an EMR. Preferred Qualifications: Associate's Degree Healthcare related field. 1 year One year experience with coding: ICD-10, CPT, HCPCS. Why Join Providence?
