Olympic Area Agency on Aging Case Manager in Raymond, Washington

Provides case management for in-home care to seniors and adults with disabilities including assessment of care needs, creating service plans, coordinating care providers; focusing on client independence. Master's Degree in behavioral or health sciences & 1 yr. paid social service experience or BA in similar fields and 2 yrs. paid social service experience; or BA and 4 yrs. paid social service experience.
