Express Employment Community Health Worker in Olympia, Washington

Express is now hiring for a Community Health Worker at a local non-profit based out of Olympia, WA. This is an evaluation-permanent opportunity. The schedule will be considered full time, 35 hours per week, Monday-Thursday 9:00AM-5:00PM, Friday 9:00-2:00PM. Occasional nights/weekends. The compensation for this position will start at $25-$25.50/hr DOE. This company offers benefits to include medical, dental, vision, and life insurance. They also offer:

  • Holiday Pay-90 hours per fiscal year, days chosen by employee
  • Vacation Pay-2 weeks/year- accrued per hour worked
  • Sick Pay- 12 days/year- accrued per hour worked

     

*Job Duties: *This organization is launching a Pathways Program, in collaboration with CHOICE Regional Health Network's Community Care Hub Care Coordination Program. Community Health Workers will provide comprehensive care collaboration services to chronically homeless families with children. CHW's will support families in establishing and achieving goals, connecting to community resources, identify housing opportunities, and ensure they are accessing any and all available resources in which they are eligible for. This position will provide services onsite at the organization's main office, shelter locations, and other community locations as requested by the clients served.

Provide comprehensive assessment and case management support to chronically homeless families with children who are eligible for the Pathways Program. Maintain a caseload of 30-35 families.

Ensure all required documentation and confidentiality/care agreements are completed and maintained within the client file.

Track the services offered to clients using Apricot, HMIS, and/or CHOICE Hub data system as applicable.

Ensure all agreements and client authorizations are returned to CHOICE within the required timelines in order to ensure contract compliance.

Work alongside families to establish goals, identify barriers that impact goal attainment, and action steps necessary to achieve goals. Keep goal plan updated and reflective of the needs and desires of the family.

Provide care collaboration across systems- mental/behavioral health, doctors, community providers, schools, etc- that support the family in achieving their goals and improving their health.

Provide ongoing case management support to assist family in achieving their goals, including identifying permanent housing options through internal and external referrals; this may include rapid re-housing, shelter, permanent supportive housing referrals, diversion, etc.

Provide direct services to assist clients in meeting their goals. I.e.: assist in completing housing applications, providing direct landlord advocacy and coordination, completing credit repair tasks, and addressing other barriers to accessing housing and/or achieving greater stability.

Meet with clients as often as necessary to evaluate progress in meeting steps in action plan, celebrate successes, provide comprehensive one on one support and advocacy to overcome challenges, and reassess and modifying goals as necessary to ensure success.

Meet with families in locations that work best for them; this may include home visits, co-location at other community organization, library, school, medical facility, etc. Complete necessary verifications for households to enroll and remain on the program.

Participate in weekly 1:1 supervision with Pathways Program Manager; attend any other program specific meetings or case staffing meetings.

Collaborate with other staff and program managers when additional support, case consultation, or creative problem solving is needed to support a family.

Review Pathways client data monthly with supervisor.

Provide culturally appropriate and accessible health education and information.
