Description The Palliative Care Social Worker (PCSW) is an integral member of the multidisciplinary Palliative Care team and provides skilled psychosocial services to palliative care patients and their families by promoting 1) coordination of care 2) continuity of care 3) achievement of consistent and/or expected psychosocial outcomes 4) patient advocacy, and 5) efficient resource management. Required qualifications: Master's Degree in Social Work or Related Field. 3 years in Social services experience with a health care related organization; experience should include a minimum of 2 years in a multi-disciplinary health care setting and specialized training in palliative care and/or hospice is preferred. Preferred qualifications: Upon hire: Licensed SW Associate Independent Clinical (LSWAIC) or Licensed SW Associate Advanced (LSWAA) Upon hire: Hospice and Palliative Care Certification, or eligible and willing to complete certification within 2 years of hire Why Join Providence?
