JOB SUMMARY of ESSENTIAL FUNCTIONS:

Adheres to all hospital Policies and Procedures and Philosophy of Care. Organizes and administers individualized, professional patient care to patients within The Recovery Center in support of medical care. Provides nursing care as directed by medical staff and in accordance with the objectives, policies, and procedures of the hospital and the State Nurse Practice Act. Participates in patient teaching and discharge planning. Participates in unit and hospital Performance Improvement activities. Demonstrates knowledge and skills required to respond to rapidly changing patient needs. Demonstrates knowledge and skills required to function on the unit. Uses an understanding of the principles of growth and development to assess each patient's age-specific needs and provide age-specific treatment and care.

EDUCATION, TRAINING, AND EXPERIENCE; CERTIFICATION/LICENSURE:

Graduate of an accredited school of nursing, and currently licensed to practice as a Registered Nurse in the State of Washington. Current Basic Life Support (BLS) certification required. Prior experience working in a behavioral health setting providing acute detoxification. Minimum of 3 years acute nursing experience with demonstrated leadership skills.

SPECIAL EQUIPMENT, SKILLS, OR OTHER REQUIREMENTS:

Ability to initiate a nursing plan of care using the nursing process. Ability to organize and prioritize patient assignments and unit activities. Ability to safely use equipment specific to the nursing unit. Excellent communication and interpersonal skills. Organizational, prioritization, and leadership skills. Demonstrated clinical expertise in behavioral health. Knowledge and ability to apply current theories, techniques, and practices of nursing within the psychiatric specialty.

WORK ENVIRONMENT AND HAZARDS:

Adheres to all State and Federal regulations pertaining to hazardous materials and BBP. Hazards include risk of exposure to communicable diseases, hazardous substances, and risk of physical injury from lifting patients and operation of equipment.

WORK CONTACT GROUP: All hospital staff, patients, families, vis­itors, medical staff, and the public.

SPECIAL PHYSICAL DEMANDS:

Must be able to lift a minimum of 25 pounds, and transfer and transport patients. Must be able to bend, stoop, push and pull. Walk and stand for most of the working day. Must possess visual, tactile, and manual dexterity.

ESSENTIAL FUNCTIONS:

  • Utilizes the critical thinking model throughout routine work activities. Provides and documents outcomes management patient care delivery. Solicits active involvement of the patient in conjunction with the multidisciplinary team members in goal planning processes. Goals and outcomes are routinely documented. Negotiated shift goals are written on the patient’s white board to promote active patient participation.

  • Demonstrates proficiency in department specific nursing skills. Completes, maintains and demonstrates competency in all unit based orientation and annual competencies per regulatory, departmental and hospital standards.

  • Uses an understanding of the prin­ciples of growth and develop­ment to as­sess each patient's age specific needs and pro­vide age specific treat­ment and care. Meets age specific competencies.

  • Promotes an environment that exhibits the hospital’s Standards of Excellence by maintaining organization and work readiness of the department, patient rooms, and patient care areas. Provides a clean, calm, orderly, and quiet patient room environment. Demonstrates attitudes and behaviors in interacting with others that are courteous, polite, concerned, friendly and respectful, re­sulting in positive customer perceptions.

  • Identifies and responds to individual patient, staff and departmental/hospital safety needs. Implements all patient safety goals and standards. Completes and documents patient checks. Provides for patient safety through preventive measures. Assesses for fall risk and implements appropriate measures. Assignments are completed in a safe and accurate manner.

  • Operates equipment per manufacturer recommendations; follows proce­dures in maintaining equipment and reporting equipment malfunction.

  • Remains proactive, flexible and supportive of team efforts to meet changing work flow activities related to patient admissions and discharges. Functions as a team member by facilitating the organization and delegation of work assignments based on patient acuity, individual staff assignments, and staff skill /competency levels. Retains ultimate responsibility for tasks and care delivery delegated to LPN and NAC personnel.

  • Ensures that job performance is consistent with:

  • EHM standard 3.1 Plan for the Provision of Patient Care (Section IV, Philosophy of Patient Care Services; Section V, Definition of Patient Care Services, RN role and functions; Section IX, Scope of Care and Servic­es Provided, Nursing).

  • Completes initial and ongoing patient assessments. Documents data per standards. Maintains complete, accurate, concise, legible documentation of patient status, nursing interventions, patient responses, and patient goals and outcomes.

  • Accountable for the formulation, updating, and revision of the multidisciplinary patient plan of care for assigned pa­tients. POC is based on the assessed patient problems, interventions, goals and outcomes. Determines the effectiveness of patient care through ongoing assessment of patient responses to interventions. Adjusts the POC and patient care to meet the patient’s changing needs.

  • Works in conjunction with the multidisciplinary team members in the assessment, development, implementation, evaluation and documentation of the patient’s discharge plan of care. Assures POC is coordinated and complete. Makes appropriate referrals. Involved the patient and family members in the discharge POC decision making processes. Ensures that the discharge plan of care is included in patient teaching activities.

  • Coordinates patient care assignment and work activities with LPN and NAC team members.

  • Organizes and completes direct patient care delivery including medication and pain management based on the identified needs and priorities of assigned patients. Assesses, plans, implements, evaluates and documents. Nursing care is individualized to meet each patient’s specific needs.

  • Communicates assessment data and findings to other members of the health care team. Reports abnormal or unexpected patient outcomes to the physician, charge nurse, nurse manager, and nursing supervisor as appropriate. Recognizes and reports risk management concerns.

  • Makes effective nursing judgments including crisis/emergency situa­tions. Remains knowledgeable in and implements emergency procedures for Codes.

  • Identifies patient's and family’s learning needs. Assesses and documents the patient’s ability and readiness to learn. Completes and documents ongoing assessment, planning, implementation and evaluation of teaching interventions and patient education outcomes. Uses appropriate educational modalities. Documents per standards.

  • Completes timely shift change walking rounds. Effectively communicates pertinent patient data. Report format includes short and long term objectives (goals) and reviews patient responses (outcomes) to the care provided.

  • Maintains the role of patient advocate.

  • Provides or assists with orientation and education of new and existing personnel and students. Effectively supports and mentors all team members.

  • Completes all department specific mandatory reading expectations. Applies information and applicable changes to job functions. Attends staff meetings.

  • Other duties as assigned
