Ambulatory Oncology ClinicalPharmacist – Providence Regional Cancer Systems: Providence is calling an ambulatory oncology clinical
pharmacist (1.0FTE) to Providence Southwest Region in Washington State.
The Outpatient Infusion Clinical Pharmacist will provide routine clinical
activities and supervise compounding of sterile preparations within the PRCS
clinic pharmacies. The Pharmacist will provide consultations to providers,
review treatment plans and perform education to patients/family and other healthcare
professionals. While participating in developing and monitoring
patients chemotherapy regimens and other supportive therapy, the pharmacist
will maintain the mission and core values of the Sisters of Providence. This is
a full time position with varying hours based predominantly in the Lacey
clinic, but with varying locations. The clinics are administered under
Providence Centralia Hospital and are located in Lacey, Centralia and Aberdeen.
In this position you will:
Conductthorough patient profile reviews to monitor for adverse events and assessthe appropriateness of prescribed therapies.
Evaluateall provider orders and verify correctness of entry in the pharmacycomputer system and computerized patient profiles.
Participatein collaborative drug therapy management.
Ensureknowledge of physiology and pathophysiology of managed diseaseconditions.
Participatein teaching of APPE students and pharmacy residents.
Ensurecorrect preparation, labeling, and delivery of medications includingcompounded sterile products.
Directpharmacy technicians in all areas of technical responsibilities includingsupervision of proper garbing, handwashing, cleaning, and asepticprocesses.
Provideeducation to patients, physicians, nurses, and other members of the healthcareteam.
Participatein protocol development, exploratory projects and committees whichcontribute to improved patient care.
Documentclinical interventions as appropriate.
Required qualifications forthis position include:
Doctorateof Pharmacy degree from an accredited school of Pharmacy
Currentregistration and license from Washington State Board of Pharmacy
BLScertification must be obtained within 6 months of hire
CompletedASHP-accredited PGY-1 Residency program or 3 years of work experience
BCOPcertification must be obtained within 3 years of hire
Preferred qualifications forthis position include:
- CompletedASHP-accredited PGY-2 Residency program in Oncology or equivalent workexperience
