Actalent RN Case Manager in Kent, Washington

Description:

The Registered Nurse Case Manager establishes a well-rounded care practice that includes: a patient assessment, care planning, implementation, coordination, and monitoring and ongoing routine evaluation that ensures medically and financially appropriate care. Responsibilities include: facilitating interdisciplinary team care coordination, intensive medical management of participants recently discharged from hospital and skilled nursing facilities. Additionally, the RN CM will work closely with the IDT Social Worker and Physician, to collect the necessary medical and social information to ensure comprehensive decisions are made which ensure the safety and wellbeing of participants. Duties include but not limited to:

  • Acts as a medical advocate for the participant, and coordinates care services with the Interdisciplinary team; Determines whether care is warranted or if IDT needs to work with family and providers to ensure understanding of benefits and risks of procedure and treatments

  • Understands and employs the Essential Activities of Case Management: Assessment, Planning, Implementation, Coordination, Monitoring and Evaluation of disease management in maintaining our participants in the community

  • Builds strong relations with participants, caregivers, families, and community providers so that communication and coordination occur in a manner that facilitates optimal care and financial oversight.

  • Conducts initial and ongoing nursing assessments of all participants; monitors health problems and health status as needed and implements the nursing care plan.

  • Ensures that documentation reflects adherence to nursing standards, interdisciplinary care plan and progress toward identified patient outcomes.

  • Contributes to the care plan by making pertinent nursing observations and participating in interdisciplinary team care conferences, staff meetings and in-services as needed; reviews charts and provides information about the necessity of care and services for specialty services; builds strong relationships with participants, caregivers, and families to ensure proper coordination of care.

  • Provides clinical leadership of chronic disease management and directs oversight of care with Primary Care Provider to ensure evidenced-based care is provided.

  • Assures the timely coordination of DME and medical supplies to meet changing participant needs.

  • Explains procedures and treatments to patients to ensure understanding of benefits and risks of procedure and treatments.

Qualifications:

  • WA State Registered Nurse license

  • Graduate of accredited nursing program-BSN preferred but would consider Associates w/experience

  • 1+ year of long-term care or geriatric nursing experience- would consider someone coming from hospital/bedside nursing as well

  • Med surge or rehabilitation medicine is a plus

  • EMR/EPIC experience

  • BLS required through American Heart Association ONLY (can get prior to starting if they don't currently hold it)
