Responsibilities

JOB SUMMARY

This job represents VMFH during initial phone contact by referring hospitals and providers with information and other hospital operations’ support. The Transfer Center Coordinator utilizes significant working knowledge of the Clinical software, to provide administrative support. This role supports the Transfer Center operations by updating patient demographics including PCP information, generating medical record numbers for new patients, creating and entering information into the hospital intake form, anticipating Intake RN needs by updating patient estimated time of arrival, requesting diagnostic imaging and confirming its receipt, and stroke call data entry. This job also includes connecting referring providers to emergency department providers for a warm handoff for incoming emergency patients The Transfer Center Coordinator provides a high level of customer service in a manner consistent with VMFH mission, goals, and service standards.

ESSENTIAL JOB FUNCTIONS:

  • Completes intake of incoming transfer requests and assists transferring facilities.

  • Answers incoming requests and assists transferring facilities based on specific patient care needs.

  • Answers incoming requests and assists transferring facilities based on their needs.

  • Generates a Medical Record Number (MRN) and FIN number for new patient visits.

  • Communicates effectively and efficiently with the Intake RN, referring facility, hospital medicine providers, and specialty providers verbally and via the EMR messaging for internal communication.

  • Connects contracted external referring providers to emergency department providers for a warm hand off for incoming emergency patients.

  • Manages all future date direct admits ensuring effective communication to facilities, sending clinics, and patients.

  • Collaborates with central patient placement to ensure bed assignments occur in a timely manner.

  • Functions as a liaison for interfacility transfers by providing regular updates to sending facilities on the progression of transfers and documents all transfer interactions in the transfer software.

  • Closes out transfer requests when completed.

  • Partners with central patient placement to expedite bed assignments.

Maintains stroke transfers database and generates reports monthly, distributing as directed.

Supports the Transfer Center RN with transfer intakes as time permits.

Qualifications

Education/Work Experience Requirements

High School diploma or GED. Minimum 2 years work experience in healthcare. Working knowledge of medical terminology.

Pay Range

$25.25 - $36.61 /hour
