Seattle Children’s Hospital Clinic Based Community Care Coordinator in Everett, Washington

Responsibilities: The Care Coordinator (CC) works in partnership with the health care team to coordinate patient care and transitional support by providing administrative and clinical support to patients and families. Assess eligibility for and assist families' enrollment in WA State medical assistance programs (Medicaid, BHP, CHIP, etc.) and financial aid. They assist with referrals to organizational and community resources as identified by the care team. Provide linkage and referral to community resources for families served by Bellevue. Participate as a member of Bellevue multidisciplinary case management team. Using culturally and linguistically appropriate methods, the Care Coordinator addresses barriers, such as transportation and health literacy, for target language speaking families and facilitates effective communication between target language-speaking families, their primary care physicians, Children's health care providers, and the community. The Care Coordinator works to increase trust between families and providers, increase self-efficacy among target language speaking families accessing and navigating the health care system, and increase effective communication between Children's providers, partner clinics, and target language speaking families. Regional full-time community care coordinator has additional responsibilities including regional site of care assignment, participation in community forums, health care for homeless families, and child-passenger safety. Serves as a resource in the area of diversity, as a partner to provide consultation and support the development and implementation of solutions to meet the needs of Bellevue Seattle Children's and community and clinical partners. Requirements: Required Education/Experience: High school diploma or equivalent experience. Demonstrated success in working with diverse populations. Demonstrated skill with computer databases, registries, and/or practice management systems. Bilingual in Spanish for Community Care Coordinator. Per the driver's license requirement noted below, must have a driving record acceptable to Risk Management. Required Credentials: Current Washington State Driver's License. Preferred: Bachelor's Degree or equivalent. Bilingual Spanish. Case management serving complex families. Min to Max Hourly Salary: $27.61 - $41.42 /hr Min to Max Annual Salary: $57,428.80 - $86,153.60 /yr Salary Information: This compensation range was calculated based on full-time employment (2080 hours worked per calendar year). Offers are determined by multiple factors including equity, skills, experience, and expertise, and may vary within the range provided.
