As an RN Case Manager, you'll be a key player in driving patient care from assessment to implementation, creating continuity of care across all phases of the Case Management Program (CMP). Certifications: One or more from the following list, including but not limited to Certified Case Manager (CCM), Certified Disability Management Specialist (CDMS), Certified Rehabilitation Registered Nurse (CRRN), Certified Occupational Health Nurse (COHN/COHN-S), or Care Manager Certified (CMC). BLS: Basic Life Support (American Heart Association). Licensure: Current, full, active, and unrestricted RN license.
