Northeast Washington Alliance Counseling Services Discharge Coordinator in Colville, Washington
*TITLE: * Discharge Coordinator
DUTIES: Primary responsibility is to evaluate the individual's support networks and systems, preparing the individual to navigate health delivery systems in medical, behavioral health care, and other social service systems as needed upon discharge from inpatient psychiatric facility. Ensure smooth transition and comprehensive discharge plans for patients and their family. Ensure individuals become linked with and access aftercare services in order to avoid re-hospitalization upon discharge.
MINIMUM QUALIFICATIONS: Successful applicants will need a Bachelor's Degree in Counseling, Psychology, Social Work, or related field - Master's Degree preferred. Additionally, you must be eligible for at least an Agency Affiliated Counselor Registration with the WA State Department of Health. Must have good written & oral communication skills. Computer literacy desirable -- clinical software used. Abstract of driving record for past three years required. Satisfactory criminal history backgrounds check results are a condition of employment.
SALARY AND BENEFITS: SALARY AND BENEFITS: $29.70 -- $39.87 per hour. Full time employees (and their families) receive $1100 per month for enrollment in company medical, dental, and vision as well as employer paid life insurance with voluntary plan options, Washington State Retirement Plans and deferred compensation plans, 12 paid holidays per year and start at 12 hours of paid time off per month.
