Providence is calling a Palliative Care Social Worker/ MSW to support our Providence Centralia Hospital in Centralia, Washington.
The Palliative Care
Social Worker is an integral member of the multidisciplinary Palliative Care Inpatient Team and will work with medically
complex patients and their families to provide supportive whole person care by
promoting 1) collaborative development of plans of care that align with the
patient’s goals 2) advance care planning 3) psychosocial and emotional
support for transitions, crises, and quality of life issues 4) patient
advocacy, and 5) identification of and linking patients to resources. The
Palliative Care Social Worker facilitates coordination and communication
between the patient, members of the health care team, and family in the
decision making process to minimize fragmentation of care throughout the
Palliative Care Services is part of the Southwest Service Area. This position will primarily work at the PCH location and will also cross train to our PSPH team with potential to work there on occasion.
In this position you will:
Coordinate the communication between all members of the health care team, patient and family in the decision making process to minimize fragmentation of the health care delivery system.
Facilitate advanced care planning discussions; intervene in crises; mediate conflicts within families; research power of attorney for health care, and guardianship; address Advance Directives; assist with available community resources and how to gain access to them (support groups, personal care homes, LTAC, etc.); assess complicated family dynamics (financial, cultural, anticipatory grief, complicated grief, etc.); and assess psychological stressors causing pain.
Required qualifications for this position include:
Required: Masters in Social Work (MSW)
Active Social Work or mental health license in Washington State.
Three years of social services experience with a health care related organization; experience should include a minimum of two years in a multi-disciplinary health care setting
Preferred qualifications for this position include:
Licensed SW Associate Independent Clinical (LSWAIC) or Licensed SW Associate Advanced (LSWAA). Hospice and Palliative Care Certification or eligible and working toward certification or willing to complete certification within 2 years of hire.
Specialized training in palliative care and/or hospice is preferred.
