Kitsap Community Resources Case Manager - Supportive Housing in Bremerton, Washington

Class: Hourly, Non-Exempt

Job Type: Full Time

Salary: $20.50/ hr

POSITION SUMMARY

Provide case management to households experiencing homelessness through our Rapid Re-Housing, Transitional, Eviction prevention, Emergency Housing, and/or ROAST programs. Client barriers may include, but are not limited to, mental health, substance use disorders, eviction history, unemployment, and medical illnesses. ROAST case managed clients will typically be experiencing comorbid mental health and substance use, chronically homeless and have multiple barriers to obtaining safe affordable housing. The Case Manager will receive referrals for case management from Coordinated Entry and actively manage a dynamic case load. The case manager will collaborate with other service providers to ensure each household is connected to the resources they desire for their success. Case management activities include establishing stability plans, budgeting, housing search, landlord mediation, document collection, benefit application assistance, crisis intervention, and more.

JOB DUTIES

  • Provide direct services to unhoused, at-risk of becoming unhoused, and recently housed individuals.

  • Provide information and referrals to assist with the stabilization process in locating and moving into permanent housing.

  • Establish knowledge of and develop positive professional relationships with community partners and service providers to provide information to Individuals.

  • Proactively reach out to and maintain communication by performing home visits (including hotels and encampments, phone calls, texts) when appropriate with Individuals.

  • Develop and utilize skills when working with Individuals including trauma-informed care, motivational interviewing, progressive engagement and strength-based coaching.

  • Help each household develop housing stabilization and goal planning.

  • Work with Individuals, landlords, and community partners to ensure all program requirements are met.

  • Read and understand program guidelines and forms required by funders and submit required reports in a timely manner.

  • Maintain client files, keep accurate case notes and keep other required records in all required data systems.

  • Maintain an integral relationship with other staff and be an active team member. Participate in a team-oriented approach including attending all required team meetings.

  • Keep immediate supervisor promptly and fully informed of all problems or unusual matters of significance and take prompt corrective action when necessary or suggest alternative courses of action

  • Seek and participate in meetings, conferences, training sessions and workshop opportunities.

  • Adhere to agency policies and practices including confidentiality, mandatory reporting, and ethical standards.

  • Accept other job-related duties as assigned.

QUALIFICATIONS:

Required:

  • High School or GED plus 2 years' experience

  • Demonstrated proficiency in the use of MS Suite products, including, but not limited to Word, Excel, and Outlook

  • Ability to pass a Washington State Criminal Background Check

  • Must have a valid driver's license, reliable transportation, and demonstrate the ability to maintain auto insurance.

Preferred:

  • Multi-Lingual

  • Experience working with individuals with high barriers to housing stability including mental health, substance use disorders, and medical illness

  • Familiar with resources for families or individuals who are low-income or unhoused

  • Understand the socio-economic systems at the root causes of poverty

ESSENTIAL PHYSICAL/MENTAL FUNCTIONS

  • Requires walking, reaching/stretching, intermittent sitting, twisting, bending.

  • Ability to occasionally lift 20 pounds.

  • Demonstrated ability to communicate successfully both verbally and in writing.

  • Receive guidance verbally and written in English.

  • Communicate effectively with individuals with a broad range of backgrounds, exhibit cultural competency and be poverty informed.

  • Requires accuracy, good communication, judgment, and the ability to remain calm and provide appropriate advice and information.
