PeaceHealth is seeking a Patient Safety Consultant for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $44.25 – $66.37. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

Performs lead role in the response to serious safety events. Accountable for the patient safety program within assigned scope of responsibility; model and develop a culture of safety; identify opportunities and support clinical initiatives to enhance patient safety. Provides leadership for Safety STOP process and the journey to 100% perfect care and zero harm.

Details of the Position:

  • Facilitates the review and analysis of all serious safety events meeting criteria and selected close calls to identify system factors contributing to error. Serves as a facilitator of the Safety STOP response team. Supports counter-measures to assure the next patient is safe. Supports operations in monitoring the implementation and effectiveness of action plans to mitigate future risks. Participates in on-call coverage to assure response to safety events within targeted times.

  • Maintains data reports and summaries of patient-safety-related occurrences and makes appropriate reports to various agencies (or Patient Safety Organizations) as required by law or determined by the organization. Meets with key stakeholders to prioritize additional patient safety concerns and determine appropriate level of review/investigation.

  • Participates in system wide meetings to ensure that lessons learned from incidents in one community are shared and enhancements to process or procedures are implemented across the entire organization to mitigate occurrence in another PeaceHealth community.

  • Serves as coach or primary data resource for individual unit patient safety representatives seeking to engage in focused patient safety projects. Encourages collaboration with other areas as appropriate. Provides education on Safety Science, including Human Factors and Reliability, to councils, units and departments as requested.

  • Conducts safety rounds in assigned areas to foster transparency and to hear from caregivers what their greatest concerns are about the environment in which they work. Coaches' caregivers on utilizing DMS huddles to escalate safety concerns.

  • Co-facilitates meetings, develops agendas and documents the proceedings of the assigned area patient safety committee. Ensures follow through on action items.

  • Assists in the development and, implementation, of educational patient safety programs for all l caregivers.

  • Works collaboratively with leadership to develop and implement strategies to eliminate harm to patients. Implements programs and services that produce measurable improvements in processes and practices that affect patient safety.

  • Proactively and reactively addresses patient safety issues utilizing data from root cause analysis, patient safety culture surveys, internal reporting systems, information from facility and system patient Safety committees, executive rounding and other national and state reports and alerts to analyze issues, determine their root causes, and implement effective and reliable solutions. Participates in the development of system-wide action plans to ensure that lessons learned are spread across the entire organization so that problems resolved in one area of the organization do not reappear elsewhere.

  • Facilitates FMEAs and OAs for processes occurring within the network. May be called upon to participate and assist with another network’s RCA/ FMEA process or system-wide events periodically.

  • Performs other duties as assigned.

What You Bring:

Education

  • Bachelor's Degree Required: in healthcare related field or equivalent knowledge and skills obtained through a combination of education, training and experience And

  • Master's Degree Preferred:

Experience

  • Minimum of 5 years Required: Experience in nursing, quality improvement, or risk management and

  • Preferred: Healthcare experience

Credentials

  • Required : Registered Nurse If a nurse or certified in a clinical equivalent field and

  • Preferred : Certified Professional in Patient Safety

Skills

  • Formal Training in root cause analysis, Six Sigma, A3 or Lean. Formal Training in TeamSTEPPS, Just Culture, Crucial Conversations and Influencer Model or equivalent (Preferred)

  • Excellent interpersonal skills (Required)

  • Excellent verbal and written communication skills (Required)

  • Ability to make independent judgments and pay close attention to details (Required)

  • Ability to analyze large data sets and design a process to identify cause and solution (may require the ability to perform analytic root-cause analysis, failure mode, and effects analysis, or to institute detailed analysis to determine the origin of errors.) (Required)

  • Ability to maintain confidentiality of highly sensitive information (Required)

Department / Location Specific Notes

Requires travel to hospitals and PHMG clinics within the network. May require travel to other networks or the system. Participation in on-call coverage of Safety STOP process required.

  • Consistently operates computer and other office equipment.

  • Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects.

  • Some time spent on site in medical/hospital setting.

  • Ability to communicate and exchange accurate information.

  • The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading.
