Provides case management for in-home care to seniors and adults with disabilities including assessment of care needs, creating service plans, coordinating care providers; focusing on client independence.   BA in in behavioral or health sciences and 2 yrs. paid social service experience; or BA and 4 yrs. paid social service experience; or Master's Degree in behavioral or health sciences & 1 yr. paid social service experience.
